Employee worked almost all of the shift, they reported during the day they were experiencing minimal urination stating heat seemed to be affecting them. Employee was taken to the office, given cool water, rest in air condition until they felt ready to return to the plant.

after changing screens on the 7x pit side screendeck top deck screens, the employee stood up and said their back was hurting. Diagnosed with lumbar muscle strain, working modified duty, return to dr 12/19
Employee was changing return rollers on C11. Employee lifted the belt to install the return roller and felt a sharp pain in neck and back.
employee was standing on bumper of pickup truck getting tool out of bed of truck. When EE started to get off the bumper EE's boot got hooked between the truck and bumper and EE sprained EE's right ankle
Employee was using a small hammer to hammer a pin in place. It appears a piece of rock broke off and struck the left upper arm of the employee, causing a laceration.
Complicated seizure disorder (not work related)
EE was rolling out a torch line from EE's service truck and turned to pull some more line and twisted EE's right knee while turning back toward the truck.

Employee was checking display lights at scale house, slipped, fell and hit shoulder on rock. Fracture, top, left shoulder
Employee was using water hose to wash underneath crusher, while bending over he felt a pain in his back.
Driller was tubing drill hole when wind blew dust into his eye. He was wearing safety glasses. He went to quarry office and flushed eye out. Still felt irritated so he went to doctor the next day. Nothing was found in eye but he was prescribed eye drops. No work time was missed.
Two employees were removing return roller on C1. When the roller cleared the hangar brackets it slipped out of their hands striking the right middle finger of one employee's hands against the concrete pad. Contusion to tendons/ligaments.
Employee was changing drill bit on power drill while it was plugged in. Employee accidentally hit power button and drill came on twisting right index finger, small cut to right thumb. Not wearing gloves. Right index finger fractured
Employee was lifting side of conveyor belt to push support wood over top of roller to install belt buckles and pulled his back muscle
Chging out stationary jaw dye trying to remove wedge from crusher. Metal blocks welded over bolt holes of wedge to prevent bolts from coming out of wedge holes. Porto power placed on back end of bolts to jack wedge out of mainframe of crusher. Pressure applied to bolts to remove wedge, metal block came off wedge, struck employee in L hand. Fracture to hand
