On April 25, 2022, at approximately 4:00 p.m., Anthowan Smith, a 41 year-old maintenance laborer with approximately 25 weeks of mining experience, was fatally injured when he collapsed and fell face-first into a puddle of water approximately two inches deep. The autopsy evidence indicates that Mr. Smith died due to drowning, complicating seizure disorder.
