While cutting stone on the big saw, employee was using a pry bar to move the stone. While he was using maximum effort the stone moved - resistance going from maximum to nothing causing his arms to be thrust out in front of him while still holding the pry bar.
While stacking stones-palletizing tumbled stone Injury reached down to pick up a stone. While picking it up. Another stone fell out of the tumbler. His finger was caught between 2 stones that cut required 4 stitches.
Injury set his parking brakes and got off his forklift to instruct his fellow workers on. How to perform their job when finished. He turned rapadily and mistepped and his face collided with the master of the forklift. Resulting in 7 stitches-3 nose-4 cheek.
While picking up sawn stone with a forklift to place on a chopper employee got off the forklift to move a stone. A small stone fell off his head and cut his foot behind his steel top. Has some bruising and required stitches. Later got infected and is still being treated for infection.
The employee reported on the 16th that he believed he was bitten by a spider on 5/13/2008 and that it has gotten worse every day. He was sent to the Doctor. The Doctor told him he could return to work with restrictions, however the employee did not report back until 5/26/2008 with release from the Doctor.
While splitting stone in the quarry, EE removed the hammer to move to another drilled hole, lost his balance. Did not fall, but dropped hammer on his foot/toe.
