Employee was on a safe rack when the flooring gave way. Employee did not fall all the way through and fall protection was in use. During the process of catching self the employee bruised the left arm and left leg.
operator was working on the journal in the Rm when EE's hand became pinched causing a small Skin Avulsion
Loader operator was dismounting. placed right leg on the ground and went to turn to the right. Right foot did not move resulting in the force going to the right knee. Doctors initial ruling is a sprain
Employee fell from a chair roughly 56" inches from the ground. On impact, majority of body wait landed on the the left elbow causing a sprain.
Installing a motor on a piece of machinery using a forklift. Employee reached under the motor to straighten the mounting bolt and the forklift dropped the motor suddenly, catching EE's left thumb.
Third party temporary laborer hit their hard hat on the frame of a conveyor in the central crushing area. The impact resulted in a neck strain. this was not reported until the 12th, at which time the laborers company (ICR) sent them to their industrial clinic
Employee was removing a guard to begin maintenance on the ball mill when EE felt EE's knee pop. Atter evaluation initial diagnosis is a knee sprain
Employee was using a pry bar to dislodge broken plow tips in RMI. They felt the job was strenuous but was alright when they completed it. They had a prescheduled day off 10/28/24 and returned to work on 10/29/24. Their back was stiff and mentioned it to the Maint. Coordinator. By 10:00 AM their back had a spasm, and they could hardly walk.
Employee working in extremely hot weather outside. EE noticed symptoms of heat stress, so EE was taken to the hospital Emergency Room, where EE was given two liters of IV fluid to rehydrate EE and then they released EE.
Employee was manually rotating piece of machinery when shoulder strain occurred.
Employee was on the Sentinel Haul Road, got out of EE's haul truck to remove a small tree that was in the roadway. EE was on unlevel surface, did not test the weight of the tree and did not use safe lifting procedures. Later in the week, employee began to feel pain in EE's abdomen and reported the incident on 4/19/2024.
Employee was handling heavy part of lab equipment
Employee had injured knee a month and a half prior falling from first step of a ladder, but thought it was minor and would go away. When pain persisted, EE was sent to doctor for evaluation. Diagnosis was knee strain.
Employee arrived at work and was getting out of EE's truck to go clock in. As EE was getting out of EE's truck, EE's left foot slipped on black ice, and EE fell out of the truck onto the asphalt injuring left shoulder. It had snowed during the night and the parking lot was covered with black ice and snow.
While removing the MyMill metal frame from the building, mechanics were rotating it 90 degrees because of limited spacing and to avoid overhead electrical wiring. The mechanic's right ring finger got pinched between the metal frame and the eyebolt connected to the motor.
Employee was handling a sharp rock at the quarry. As EE was turning the rock over, EE received a cut to EE's right hand
Maintenance supervisor was disconnecting an electric forklift battery power connection. Employee came in contact with battery frame and received shock on the right wrist. Electric shock ran through their upper body to left hand. Employee was transported to the ER for evaluation and was released.
Electrician was descending the stairway. EE had one hand on the handrail, but was carrying a pipe in the other hand. EE missed the last step and as EE stepped down EE injured EE's right foot and ankle. EE heard a loud popping sound
Employee was moving an empty water dispenser (approximately 33 lbs.) and felt a pain in EE's upper left chest/rib area.
Employee developed a rash on right arm. Believes they were exposed to biocide. Previous exposure to biocide may have made them sensitized. Employee went to doctor and received medication.
Loader was loading a bulk truck. While handling a vacuum hose, the loader experienced a pull and pain in right shoulder.
During their shift, employee got something in their eye. After the shift, they went to medical facility and they removed foreign material from between their eye and eyelid. They received medicine for the eye and reported to work for their next shift.
Employee was stepping down from the excavator, EE's foot slipped, and EE hit EE's right knee on the excavator tracks.
Employee was pushed and as EE stepped back, EE stepped down missing a step, lost EE's balance and fell on EE's right knee sustaining injury to multiple body parts.
Hose for test procedure stretched across walkway. Employee tripped on hose and impacted EE's elbow.
While tightening lug nuts on a 992 loader the employee was using a 1 inch impact and EE noticed that the ring holding the safety pin on the extension was coming out and by the time EE could react the pin had come out and caught EE's wrist, causing the cut. A fellow employee helped administer first aid to stop the bleeding and bandaged it up.
Employee reported eye was irritated when EE woke up. Does not remember a specific incident, but assumes it happened at work the day before (6/20)
Employee stated that EE's ankle hurt at the end of the day. Unsure when or where EE might have injured it. Employee sought medical care on EE's own. Accident not reported for 4 days. Employee was examined and diagnosed with ankle sprain. Was put on restricted duty.
Tripped on grating EE was installing and fell onto left wrist, resulting in a sprained wrist.
Employee slipped on ground and turned ankle. Restricted duty only for 48 hours
Employee was using a Kenworth water truck to provide dust suppression and ran over an unseen rock causing the truck to swerve and the operator felt pain in EE's neck from the movement. The operator went to the company clinic and was placed on modified duty for 1 week till re-evaluation
Forklift operator was loading a contract van truck and they had a miscommunication and the driver started to reposition the truck by pulling forward at the same time the operator was returning to start loading. The forklift did not fully enter the truck and the rear tires of the forklift did not make it into the trailer causing a jolt to the operator and a sore back.
The geologist was collecting drill hole samples on a quarry bench and did not see a rock and stepped on it causing ankle to twist. This employee filed a report at that time and thought it would be ok. After some time the discomfort returned and the employee went to visit the DR. and the subsequent modified duty was issued.
Employee was driving a ground rod into the dirt using an electric hammer. The hammer slipped off the tip of the rod causing employee to fall forward. Employee fell into the ditch and landed wrong on foot.
Employee was working on mill reassembly. Noticed a pain in EE's back. Reported four days later when it did not improve.
Miner stacked pallets 20 high for use in packaging supersacks. When they reached for the top one a foot got caught under the pallet stack and when they went to step back was unable to. They fell back on to the roller conveyor and sustained a back contusion and sprain of ligaments of thoracic spine.
Employee was changing filter socks at end of shift. As EE raised the socks, EE felt pain in shoulder. Did not report for a week. Was seen by doctor on 1/20 and given an injection. X-rays showed no visible issues.
At 1527 on 10/29/2020 was notified commercial motor vehicle driver was found collapsed/unresponsive at upper load out scale. EMS arrived onsite and took over CPR. At 1655 per doctor on duty at Medical Center driver did not survive. Driver suffered a heart attack that was fatal due to natural causes. Mine condition and equipment were not involved.
Employee was pulling on a pump suction hose. When EE straightened up, EE felt a pain in EE's back. EE went to the ER, where EE was given two injections and two prescriptions. EE reported for EE's next shift at full capacity.
Opening driver side door of Cat 777 haul truck pulled/strained lower back.
While removing starter on drill, EE dropped socket down to belly pan of drill. EE crawled under drill to loosen belly pan to retrieve socket. EE loosened bolt too much, therefore the belly pan dropped down on one end striking employee on the head. EE had temporary swelling, dizziness and headache. EE was examined at hospital and released back to work 8/19/2019.
Employee was attempting to load feeder, reached to top of feeder and felt a strain in right shoulder.
Employee worked for about 30 mins packaging product in super sacks then started to clean for 60 minutes. EE felt light headed & went to get a drink of water. Went back to work. EE felt light headed still so EE stood in front of a fan. EE was then helped to the break room. EE felt overheated. The temperature outside was high of 100. EE had hiked the previous day. EE was dehydrated.
During a bulk truck loading and packaging Temporary worker was kneeling on the top of the truck closing the truck hatch after finishing bulk load out. When the employee stood up EE felt a sharp pain in the right knee. Kneeling to close hatches and add seals is a normal practice. The temp worker was driven to the hospital and it was determined that EE had a right knee strain.
Securing super sack on spout. Cord snapped and employee lost balance and fell into beam.
Manager tripped and fell and injured ankle on 6/21. EE reported as a near miss, thinking the injury minor. After a three weeks and the ankle did not feel any better, ee went for medical care. Doctor diagnosed strained Achilles tendon and put ee on light duty.
Exposed sharp edges. Lack of safety behavior. Unavailable nylon zip tension tool.
Employee started to pick up pump suction bowl, felt pain in back.
Employee had taken pipe apart. In attempting to move pipe to the side, it fell and caught the end of employee's left index finger between the end of the pipe and a flange on the pump, cutting off tip of finger.
On 7-13-2017 employee reported lower back pain as a consequence of placing empty bags on loading packer spout.
