Showing all 31Employee was putting a ladder in the bed of the truck, when one end of the ladder hit the toolbox causing EE's thumb to get caught between the tailgate and ladder. This caused a laceration that required 4 stitches.
Employee was standing in area of work being conducted on gear box. When maintenance team member was hitting gear, a small piece of metal broke off and struck employee in left forearm area causing a small laceration.

Employee was cutting a rag with a utility knife to cover face from wind and didn't have gloves on. The knife opened up and cut employee's right hand.

Stationary jaw die bolts were loose cut out old bolts with Air Arc wasn't wearing proper PPE and the slag rolled down arm into glove opening causing burn to right wrist.
Employee carried buckets or stone up some steps and spilled some stone on to the steps. On the way back down the employee slipped on said stones and fell on the steps injuring EE's low back on the steps.
Working on a wheel, hit ring with hammer and knocked it off and EE tried to grab it and it cut EE's right pinkie finger.

Employee removing bolts from belly plate of a wheel loader and the torque multiplier on the wrench slipped making employee loose balance employee felt a pull in the groin area.

Employee from American Tire company was at our site breaking beads on a tire on our equipment (2872). The bead breaker smashed the middle finger on right hand between the piece of equipment and the hydraulic pusher resulting in 20 stitches. The finger was not broke and no ligaments are involved.

Employee was making sure Drill Press drill bit was securely tightened. Right hand slipped causing contact with the drill bit and employee cut finger on right hand needing 3 stitches.
Employee was assisting with maintenance and walked up a ladder to hand off a wrench. When EE was stepping back down EE felt pain in lower back.
Employee was replacing V-belts on the dry screen feed conveyor and he was trying to loosen the speed reducer up so it would make it easier to slip the V-belts over the pulleys. He was using a pry bar approximately 30" long to loosen up the turn buckle when the bar slipped out he smashed his finger between the pry bar and the hand rail on the man lift.
Removing bed liner plate from haul truck with cutting torch. As he was cutting the plate with the torch, the plate flexed and hit his welding helmet and knocked him back onto the ground.
Working out of a man lift basket. Harness protection was used. Removing old idlers with new ones. Body in awkward position to do so. Felt pain in shoulder then down right arm.
Employee was removing rock from a conveyor. A large rock fell from the conveyor to the ground then ricocheted off another on the ground and hit employee's left leg.
Employee was driving retaining pin form hydraulic hammer. A piece of rock was holding retaining in place broke off and impacted right eye.
Employee exited haul truck to remove rock from haul road. Employee picked up rock (approximately 15lbs) and carried it to the side of the road. As the employee was tossing the rock he heard and felt his right knee pop.
Wrench mashed small finger, cut botom of first knuckle on right hand.
Employee opened engine access door during inspection and pulled lower back.
