Employee was cutting a zip-tie with an utility knife. when the knife cut through the zip-tie, it continued and cut their left thumb. They was received about 3 stitches.

Miner was cutting conveyor belt fabric with a utility knife and cut rt. leg.

During setting up of crusher and fine tuning the production specs, employee began complaining of sore forearms. After switching to operating a loader they didn't get much better. It appears they had this condition prior but it resurfaced. After seeing doctor, it was diagnosed as a form of carpal tunnel in the wrists.

After a long shift operator was walking the dozer down the access road from the pit and while trying to make the turn at a switchback got the tracks too far over the berm and slid down and rolled dozer down the slope, it rolled once with a slow slide.

Fly rock from a blast flew over stockpile and hit employee in the leg. Rushed to nearest hospital and diagnosed with broken rt. leg.

Employee was tightening a pressurized hydraulic fitting to stop a drip leak. He had barely touched his wrench to the fitting when it gave way releasing pressurized fluid. The crusher was NOT on/powerized but has contant pressure in the valves where the fitting was. The pressurized hyd. fluid shot out and cut his arm and neck.

HE WAS CUTTING THE STRAPS OFF OF STRAW WADDLES WITH UTILITY KNIFE.

Employee was disassembling brace for a conveyor belt legs when the brace swung down and hit his foot after he loosened one end.

It was a rainy day and the ground was muddy which led to the steps on the ladder being slippery. Employee banged his leg on the attached ladder while descending.
