Over-exertion (Not Elsewhere Classified)

During setting up of crusher and fine tuning the production specs, employee began complaining of sore forearms. After switching to operating a loader they didn't get much better. It appears they had this condition prior but it resurfaced. After seeing doctor, it was diagnosed as a form of carpal tunnel in the wrists.

Caught in, under or between a moving and a stationary object

Employee was helping to open front gate of mine when it was stuck due to accumulation of snow and ice. As it broke free, the employee's hand was on the top framework of the gate and the gate roller rolled over EE's right ring finger.

Over-exertion (Not Elsewhere Classified)

EE was stepping down from a lowboy trailer when he began to slide down a short embankment that he was parked next to. He slid only about 12 inches but when the sliding stopped, his weight shifted on his downhill side leg causing pain in his right knee. He was able to work until it was not getting better and had surgery. Released to full duty as of 11/16/2015.

Tom was tightening a nut on a bolt when the nut spun freely and cut the palm of his hand. He was holding on to the nut when it happened.

Fall to the walkway or working surface

While working in the shop, he tripped on 4 x 4 dunnage and jammed his shoulder trying to break his fall. He has continued to work until last week when he kept complaining of discomfort. He finally went to see a doctor and was diagnosed with a slight tear of a tendon. He underwent surgery this past Friday, February 8th, 2013.

Struck by falling object

Employee was pressure washing a heat muffler shield off of a D8 and while he was turning it around it slipped through his hand and sliced his tight thumb. He received 6 stitches and was back to work his next scheduled work day. Because he is attending college, his next scheduled work day was two days later.

Over-exertion in lifting objects

Employee was shoveling dirt and strained lower back muscle.

Employee stepped onto uneven ground and turned his left ankle, apparently spraining it.

Struck by falling object

Grinding on the underside of a truck trailer while on his back and metal particle got in his eye. After flushing the eye and getting the particle out the employee finished his shift with no problems but the next day still felt the eye was bothering him. He went to clinic and got an antibiotic for a scratch on his eye. After the visit to the clinic- he returned to work.

Struck against stationary object

Operator stepped on a rock and strained his right knee while trying to get off loader ladder/steps.

Fall from machine

The driver for Lynden Transport (vendor) was unloading a conex he was delivering to the shop. When he tried to undo the chain binder, the release of the binder pushed him backwards off of the trailer. He landed on the ground and on his back. Per the vendor's representative, he may have a bruised back. This driver is not an employee of ours.

Miner was helping remove a wheelhouse from tugboat in the shop area. A small mobile crane was being used to hoist it off. The miner was using a prybar to loosen a stuck corner of the wheelhouse. When the wheelhouse finally cut loose the prybar jerked forward smashing left middle finger between the prybar and the tugboat. Injury was not reported to employer until 5/14/09.
