Water pipeline blew a hole near booster pump approx. 5' deep and 4' wide. Employee was standing near break in pipe while waiting water pressure to decrease to make repair. Bank of hole sluffed off and employee fell into hole resulting in injury to their right ankle.

Showing all 7Employee was performing maintenance on 870 transmission hydraulic pump. While positioning pump on service truck bed, pump began to roll to employee's right. Employee attempted to stop pump from rolling which resulted in employee's right ring finger being pinched between pump and vise mounted to service truck bed. Employee required 15 stitches to right ring finger.
EE was struck by 5 lb hammer head while removing coupler from pump shaft. Alternating striking coupler & other EE's hammer head came off the handle striking injured EE in pelvic region. Injured EE didn't seek medical attention on day of injury. Sought treatment on 7/27/20 & was referred to a specialist. Contusion to the pelvic region above the private area was the diagnosis.
While positioning a sheet of 5'x13' 1/4" plate steel to weld, the injured employee became momentarily trapped underneath it when the steel came off of the hook used to lift it into position and fell on top of EE.
EE was getting anchor cable for dredge out of the silted sand. EE was guiding the cable on the side of the work boat in the proper cable guides. Cable became wedged and EE used pry bar to get cable out of guide. The river current moved the boat and cable came out and EE's hand was pinned against handrail with the pry bar and cable.
Employee was stepping down from scales platform in office and strained right knee.
Employee was prying on rock in oversize chute with a bar and strained his chest muscle.
