Employee was trying to throw the flop gate at the direct ship hopper, when the chain ratchet free spooled letting arm of the flop gate pinch his left hand between the flop gate arm and beam causes cuts and a fracture to his index finger of the left hand.

Operator lost engine control of fuel/lube truck on decent of haul road at the end of the shift. Operator veered the truck against a highwall to stop it. Operator struck his knees against the dash and forehead on the windshield. minor bruising incurred

Burning paint off of cutting edges, hit spot that must of had rust under paint. It popped off putting something into eye.

Operator was dismounting from Komatsu Excavator when his foot slipped and he fell resulting in a bruised left buttock

Operator was dismounting and mounting a Volvo articulating truck throughout his scheduled shift. Later in the evening, EE noticed pain in his knee and notified supervisor via text message that evening and sought medical tratment the following day.
