While driving a loaded articulated haul truck on a well traveled haul road during heavy rain, the back end of truck slid off maintained road onto rough terrain. EE drove truck back onto maintained road. EE complained of back and neck pain in the day following incident. Diagnosed with back sprain and placed on restricted duty.
Employee began to experience pain to left side lower back and rib cage. Employee thought it was from shoveling wet material(mud &/or rock). EE was diagnosed with strain to left lower back/rib cage.
Worker stepped on a rock and sprained workers left ankle.
Loader operator hauling bucket of rock from pile to feed crusher rock fell out of bucket (L) front tire ran over rock: (R) rear tire ran over rock; operators taros whipped left to right while restrained in seatbelt operator, complained of back pain 7 days later (10-99-15) treating physician prescribed pain medicine. Operator released to return to work 10-19-15
While cleaning mud from the back of the JCI 1400 Portable Screen chute with a 'L' shaped bar, the bar slipped when the mud released smashing 'R' index finger against the edge of the chute.
