While driving a loaded articulated haul truck on a well traveled haul road during heavy rain, the back end of truck slid off maintained road onto rough terrain. EE drove truck back onto maintained road. EE complained of back and neck pain in the day following incident. Diagnosed with back sprain and placed on restricted duty.

PIECE OF METAL IN EYE - NOT SURE HOW IT GOT THERE. HAD BEEN GRINDING ON METAL WITH HAND GRINDER EARLIER IN DAY AND WAS WEARING GOGGLES WHILE GRINDING. DID NOT FEEL ANYTHING IN EYE UNTIL LATE AFTERNOON.
