Employee was attempting to slide a drain grate with EE's hands reaching into the grate. The grate moved and EE's left index finger was pinched between the grate and a metal tab holding the grate down resulting in a laceration requiring stitches.

Miner was throwing away empty grease barrels into dumpster. Barrel ended up slipping out of grip of miner, which resulted in a cut to the finger. Miners tried to cover to stop bleeding for 30 mins. Resulted in taking to Hospital.

Employee was removing a pulley from gearbox shaft when employee was struck in the face with the pulley resulting in chipping a tooth.

Employee was using a grinder to cut a bearing off a shaft when debris got in eye past face shield and safety glasses. Employee did not feel any irritation in eye until the following day.

Employee bent down to move a rock when they strained their back. Injury occurred on 11/4/22 and employee sought medical attention on 11/7/22

Employee twisted ankle when the gearbox of the shaft/bearing employee was working on began to rotate due to being off balance and employee tried to catch it

Employee was pouring concrete and discovered a hole in boot allowing concrete to contact skin and causing a burn.

Employee put foot on rocker panel of truck while attempting to climb inside. Employee's foot slipped off and struck ground spraining employee's right ankle.

Employee was using a bar to pry on a scraper. Bar slipped and employee felt a pop in right shoulder. Employee reported the incident and said that the shoulder was sore but fine and did not want to seek medical treatment. 2 and 2 days weeks later on 2-13-20 employee decided to seek treatment. No clear diagnosis was given, but employee did receive restrictions.

Employee kneeling to tag a rail car and slipped on hard snow/ice. Sprained left ankle.

Employee was working with another section on 10 Jan 2020 and noted sore back. Several screens decks and screens for cleaning the sand were removed and reinstalled and EE noted some soreness after the work ended. Injury was not reported to Operator until 18 Jan 2020 after employee visited the doctor.

On 8 Jan 2020 the employee was working in the Dry Plant area and individual tripped over EE's own feet and when EE fell forward EE placed EE's left hand out and palm of hand impacted a small rock on the ground. The rock made a cut into the hand. Several stitches were given for the laceration.

Employee felt discomfort in eye on 1/6/2020 after repeated flushing with no relief on 1/7/2020 employee reported discomfort to supervisor. Employee was seen at eye clinic where a small piece of metal was removed from the eye.

Employee was making a rail switch and strained right arm. Employee sought medical attention the following day and was given restrictions.

The Employee was working to unplug a head chute. While chipping rocks away with a 5' pry bar, the bar became stuck. As Employee was pulling the pry bar free the bar quickly dislodged. The Employee was positioned directly above the bar and the end of the bar struck Employee on the left side of chest.

Foreign object in eye required removal and prescription.

While drilling the drill bit got caught twisting hand and pinched against side pipe.

In a cold E-house, the employee was laying on EE's back on the floor of the building to strip wire that would be used to land. The employee was cutting towards self, the blade slipped off, cutting through EE's glove and sliced open EE's left thumb.

Employee was checking belts and slipped on ice resulting in hitting head on the ground. Employee has previous medical non work related history with head injuries, and is off work for continued evaluations.

On 17 Jan employee was grinding a piece of metal, a particle went into right eye. EE was wearing safety glasses & face shield. On 18 Jan EE went to ER at 2100 hrs, diagnosed with viral conjunctivitis. On 21 Jan EE went to an eye doctor, as eye was still irritated. Second doctor identified a foreign metal body in right eye. Operator was not notified of injury until 23 Jan 2019.

Cage from baghouse slipped through employee's hand causing a laceration

Employee was climbing a fixed ladder located outside and the wind blew sand behind EE's safety glasses and into EE's eye. A grain of sand became lodged in EE's cornea and had to be removed by a doctor. The doctor prescribed an antibiotic ointment.

EE was climbing a ladder and felt left elbow pop when EE got to the top. EE has been diagnosed with tennis elbow.

Cutting a piece of plywood with circular saw and a piece of sawdust entered right eye around edge of safety glasses.

Employee attempted to remove sand from a 1" pin hole by sticking a finger in the hole while another employee was sliding the tow bar into the equipment. The employee's pointer finger was subsequently pinched in the hole causing a laceration that required stitches.

Fire was observed on wash tower 2.5 hours following welding on the collar of a feedbox. Fire was extinguished 43 minutes after it was noticed.

Employee was using a tinker bar to move a piece of equipment into place, which resulting in a strained back.

Employee was walking on top of a rail car when fall protection device became snagged on the supporting structure. This caused employee to be stopped mid stride while stepping from one car to the next. This caused employee to lose balance and fall between the cars. Total fall before arrest by retractable lanyard was approximately 4 feet.

Individual was shoveling around 320 silo area hit an uneven piece of concrete it stopped the shovel abruptly and jammed thumb. Employee reported after supervisor noted a hand brace on 9 June 2018. Doctor's note on 11 June noted left thumb contusion.

On the above date and time, EE was operating a dozer on a dumpsite that sloughed off into an impoundment. EE was trapped inside the dozer for about 2 hr. 17 min under water.

While employee was replacing a guard the employee's finger was pinched between the guard and a hand rail.

Employee pinched right little finger between a scraper and a hopper wall while trying to scrape frozen sand out of hopper. The finger was first treated with first aid. Two days later the employee was seen by a doctor and there was a slight fracture to the finger tip. Employee was able to return to work with no restrictions on next scheduled shift.

Employee opened a cap on a radiator while it was still hot. Steam and coolant sprayed out on the left arm and face. Employee was taken to the hospital. Employee has 1st degree burn on the face and 2nd degree burn on the left wrist.

A 289D Cat Skid steer belonging to Gerke excavating was working on property owned by Hi Crush Augusta when it suffered a mechanical failure that resulted in said piece of equipment catching on fire. An apparent hydraulic line failure allowed oil to come in contact with the exhaust and/or turbo of the skid steer, causing a fire to start.

Contractor was notified of injury on 18 Jan 2018. Employee was grinding outside and welding on scalper tower floor entire shift on 17 Jan, noticed eye irritation around 2pm. Irritation persisted and employee visited hosp urgent care on 18 Jan. Employee sustained arc flash burn to right eye.

EE was swinging sledge hammer, ee slipped and hit hand on flange and dislocated socket on right hand/pinky finger

Employee was using a cutting torch to cut through 1/4" steel on a UHMW-lined conveyor chute. The UHMW ignited and burned inside the chute, which caused the conveyor belt at the top of the chute to start on fire. Employee was unable to extinguish the fire with a fire extinguisher. The fire department was subsequently called to extinguish the fire.

Employee circumvented accepted practices for adjusting feed to the head chute of "B" 80 by climbing on a shield rather than using a ladder. When employee was getting down employee felt a pain in right ankle

The employee was standing on the floor and turned EE's body to reach for a pail when EE's knee twisted and popped. EE was not holding anything at the time of this non-contact injury.

Employee had hook from lanyard hooked to the shoulder strap of a harness while not in use. While attempting to remove hook from shoulder strap to put in use the employee removed the hook and hit themselves in the mouth with the hook.

Employee was clearing a frozen block of sand from a conveyor belt. While moving the block of sand it slipped and the employee's finger was pinched between another frozen block of sand.

While installing speed sensor portion of BC-9A conveyor tail pulley guard, individual hit guard against frame of conveyor to remove frozen sand (guard was placed on ground during maintenance) and cut right index finger approximately 1/2" in length. *Received 4 stitches along joint of index finger.

The employee was descending a metal stair way and misjudged the last step thinking ee was at the ground. The employee stated that ee was looking around the plant when this happened and not fully focused on the stairs. It was dark out and ee may have cast a shadow over the bottom stair as ee descended.

Employee reported 3 days after date of injury with work restrictions for a sprained right shoulder.

Employee was using a hook to shake a plate press when the hook slipped off the press causing the employee to stumble backwards and strike their back against an I-beam.

Employee experienced shortness of breath and chest pains after having climbing down from a silo. Employee was given a ride to the ER where the condition was subsequently diagnosed as a heart attack.

Employee was using a hand grinder to cut a section of chain when chain was pulled into grinder employee lacerated tip of finger.

Employee was cutting a hose with a utility knife and cut middle finger requiring 6 stitches.

Employee received UV burn to eyes from looking at another employee welding

Employee rolled ankle when ee stepped on a Rotex rubber ball while collecting a plant production sand grade sample.
