Employee was attempting to slide a drain grate with EE's hands reaching into the grate. The grate moved and EE's left index finger was pinched between the grate and a metal tab holding the grate down resulting in a laceration requiring stitches.

Miner was throwing away empty grease barrels into dumpster. Barrel ended up slipping out of grip of miner, which resulted in a cut to the finger. Miners tried to cover to stop bleeding for 30 mins. Resulted in taking to Hospital.

Employee was removing a pulley from gearbox shaft when employee was struck in the face with the pulley resulting in chipping a tooth.
Employee was using a grinder to cut a bearing off a shaft when debris got in eye past face shield and safety glasses. Employee did not feel any irritation in eye until the following day.

Employee was working with another section on 10 Jan 2020 and noted sore back. Several screens decks and screens for cleaning the sand were removed and reinstalled and EE noted some soreness after the work ended. Injury was not reported to Operator until 18 Jan 2020 after employee visited the doctor.
On 8 Jan 2020 the employee was working in the Dry Plant area and individual tripped over EE's own feet and when EE fell forward EE placed EE's left hand out and palm of hand impacted a small rock on the ground. The rock made a cut into the hand. Several stitches were given for the laceration.

In a cold E-house, the employee was laying on EE's back on the floor of the building to strip wire that would be used to land. The employee was cutting towards self, the blade slipped off, cutting through EE's glove and sliced open EE's left thumb.
On 17 Jan employee was grinding a piece of metal, a particle went into right eye. EE was wearing safety glasses & face shield. On 18 Jan EE went to ER at 2100 hrs, diagnosed with viral conjunctivitis. On 21 Jan EE went to an eye doctor, as eye was still irritated. Second doctor identified a foreign metal body in right eye. Operator was not notified of injury until 23 Jan 2019.

Cutting a piece of plywood with circular saw and a piece of sawdust entered right eye around edge of safety glasses.
Individual was shoveling around 320 silo area hit an uneven piece of concrete it stopped the shovel abruptly and jammed thumb. Employee reported after supervisor noted a hand brace on 9 June 2018. Doctor's note on 11 June noted left thumb contusion.
Contractor was notified of injury on 18 Jan 2018. Employee was grinding outside and welding on scalper tower floor entire shift on 17 Jan, noticed eye irritation around 2pm. Irritation persisted and employee visited hosp urgent care on 18 Jan. Employee sustained arc flash burn to right eye.

While installing speed sensor portion of BC-9A conveyor tail pulley guard, individual hit guard against frame of conveyor to remove frozen sand (guard was placed on ground during maintenance) and cut right index finger approximately 1/2"" in length. *Received 4 stitches along joint of index finger.
Employee rolled ankle when ee stepped on a Rotex rubber ball while collecting a plant production sand grade sample.
Miner was walking down provided stairs and using handrails in railcar preparation area and upon stepping off of last step to the ground rolled right ankle. Miner fell to the ground and continued work for several more minutes and then reported injury.

Individual was stepping off railcar onto load out platform and right foot caught edge of platform and fell forward scraping right arm and left hand. Majority of impact occurred onto right hand resulting in strained right shoulder.
Individual was priming a liquid floc line and removed priming cap to allow flow increase, when cap was removed the Hydriclear 9080 flocculant and water mix sprayed out. Due to pump and priming cover height at approximately 1 foot above the floor, this allowed the floc to spray upward under EE's safety glasses into both eyes.
Initial injury occurred on 6 Jan 2016 and not reported to operator supervisor until 11 Jan 2016 after employee felt they needed to see physician for groin pain. Individual was removing sand from work area with a large shovel on main floor of 320 silo when initial discomfort was noticed. After the 5 days, the individual felt they needed to visit doctor.

Injury was reported to operator on 30 Sep 2015. Individual was working in the Rail Load Out area and was opening a grain style topped railcar door. The door was 12 to 14 feet long and when opening the door he hurt his lower back muscles.
Injury was reported to operator on 13 Apr 2015. Piece of foreign metal debris went into left eye while working on a bearing replacement on bucket elevator, due to grinding and windy conditions. Did not immediately notice piece of debris in eye and worked through entire shift.
While operating chain hoist hook to pull a piece of worn steel (appx 150 lbs) out of a headbox chute, the hook end attached to anchor point rolled over once tension was applied by EE, pinched left index finger tip between the chain hoist and anchor point. Designated person working with EE assisted in relieving tension off of chain hoist hook allowing him to remove his finger.

While being tied off onto rail fall protection system, as he stepped across between 2 railcars the supplemental restraint locked and pulled him off balance. He fell slightly backwards between the cars, approximately 3 feet. He alone stepped back onto the railcar and reported incident.
While preparing to install a belt scale, he started having back pain that progressively worsened. Unidentifiable as to what caused injury, possible bending or twisting.
Two employees were working on a bearing installation and while tapping the bearing on, a small piece of metal came off and went into the individual's top right forearm area.
While sliding a small piece of 1/8"" steel plate across shop floor the plate all at once stopped and the force used to push the steel continued through causing individuals left thumb to bend backwards. No open wound occurred.

While standing on the running board of the maintenance truck, employee was observing a head pulley on a feed hopper when his right foot slipped off the running board. The right foot hit the ground first causing it to roll over. Running board surface in conjunction with sand and standing on tip of toe caused slip.
