Showing all 10Operator was bolting up 4540 heading. He went to repair a leaking hose when the right rib failed resulting in a buried bolter. He was able to escape over the top of the material.
While spraying shotcrete the dry pot stopped. The miner stepped away from the face as the dry pot was being worked on. The miner briefly pulled his attention away to check the progress of the dry pot when a rock rolled off the sloped face on to the muck pile and down to the flat. The rock continued rolling in to the miner's right leg resulting in a contusion.
While being loaded, the truck driver was shaken side to side causing pain in the left of his back.
While trying to align striker bar to drill steel, the boom on drill became in a bind, when it came loose from the sill it swung to the left striking his hand (left hand middle finger).
The hammer was inspected prior to use. Was not loose or damaged. Was used several times prior to incident. Hammer had a wooden handle & was 2 years old at least. After looking at the handle after incident, the handle looks to have shrank, causing the head to slip off and strike the employee in the head. Laceration to head 11 stitches. Full release.
An electrician was testing the power center for an 800hp, 4160v pump. His meter was undersized and became damaged by high voltage and carried a current. The probe arced to the structure and an arc flash resulted causing burns to the electrician. No electricity was contacted by the injured.
Injured was positioning a wood frame door and stepped back slightly for balance. At that point a forklift's right rear tire rolled onto/against his foot.

On return of day shift, the heading was discovered by inspection to have had a roof fall.
