Employee was operating a haul truck that became stuck in mud. To recover the vehicle, a chain was connected to a dozer. The chain broke under load and struck the operator through the windshield. The incident resulted in medical treatment for facial and head injuries. Damage to equipment was limited to the broken windshield.

Employee was unloading belt conveyor components from a trailer when EE stepped over an I-beam. When EE's left foot contacted the ground, EE's left knee buckled. The incident did not involve any equipment or compliance issues. It also did not involve job skill, proficiency, training, PPE or protective devices.

While working on a water line repair the employee stepped on uneven ground and fell curling EE's ankle. Employee had trouble walking and asked to go to the hospital. Employee was found to have a sprained ankle from the incident.

Employee was attempting to straighten piece of metal tubing by sitting on it. He placed one end on a piece of equipment, the other end on the ground. When he sat down, the tubing rolled and he fell to the ground.

Employee reported that 5' X 6' 3/8 diameter screen he was handling fell onto his left foot, behind the steel toe. The screen pierced his boot and punctured his foot requiring stitches.

EE had removed parts from a Caterpillar 980 loader and was pressure washing them. He had brushed a cleaning agent on the parts, and must have gotten some of the liquid on his hands. He developed a rash on his hands and thought he had poison ivy, until it did not get any better.

Employee reported that while inspecting a screen sensor, a piece of metal flatbar that he was holding came in contact with a vibrating screen causing the flatbar to hit his right shin. Injury required 2 stitches.
