Fall to the walkway or working surface

Employee was attempting to straighten piece of metal tubing by sitting on it. He placed one end on a piece of equipment, the other end on the ground. When he sat down, the tubing rolled and he fell to the ground.

Absorption of radiations, caustics, toxic and noxious substances

EE had removed parts from a Caterpillar 980 loader and was pressure washing them. He had brushed a cleaning agent on the parts, and must have gotten some of the liquid on his hands. He developed a rash on his hands and thought he had poison ivy, until it did not get any better.

Crusher head being removed from crusher by crane swung, crushing employee's hand against a support bolt. Crushing injury to middle and ring fingers of right hand.

Fall from machine

Employee lost his footing while dismounting a Cat 960 wheel loader. He fell to the ground catching his right arm under his body, injuring his right shoulder. Injury has been diagnosed as a damaged right rotator cuff, to be confirmed by a future MRI.

Caught in, under or between a moving and a stationary object

Injured employee was helping re-install a speed reduction gearbox onto a conveyor drive. The gearbox slipped trapping the employee's hand between the gearbox and an angle iron support. Employee suffered a crushing injury to his right index finger. Injury was later determined to involve a fracture of the right index finger.
