EE was walking towards power center. EE slipped and fell, left leg twisted behind EE. The weight of the EE's body landed on left leg.
Logic Box on hoisting car Burnt up. This is on our secondary escape way.
EE was operating track mounted mobile equipment. EE was using the sanders to disperse sand on the track to obtain traction. EE caught his left thumb between the sander bar and roof of mine causing a laceration on EE's thumb.
EE was working on continuous miner. A piece of draw rock fell from the top striking the EE in lower back.
EE was making splice in S/C cable. Knife slipped cutting EE on the back of the right thumb. EE received seven stitches.

EE was changing hub on roof bolter machine. EE was driving stud bolts in hub. EE hit his left hand with a hammer causing a small hair line fracture.
While bolting top, piece of steel hung in hole. Tried to pull piece of steel out of hole and strained left shoulder.
EE was operating the C/M. EE was struck by a piece of draw rock on the right leg.
EE was lifting on bottom roller to train belt. EE felt a strain in lower back and right hip. This became reportable when EE started missing work.
A roof fall was found 8 breaks inby surface in intake entry No.7. Survey Station 13831. This fall does not impede travel or ventilation.
EE was fixing a slip roller on conveyor belt. EE slipped and fell hitting his mouth which resulted in the EE breaking a portion of his tooth.
A non-injury accident occurred at this mine at 2:18 P.M. The slope hoist brake car sustained damage to the PLC control system and would no longer function in full capacity. All miners were removed from the entire underground portion of the mine.
Top of hydraulic jack ruptured blowing hydraulic oil on EE's upper body. A portion of the oil went into EE's eye. EE refused an ambulance but later went to the doctor on his own.
EE had put last row of bolts up. The pin crew had backed up to install a cable bolt. EE was trying to remove a broke piece of steel out of drill pot and a piece of rock measuring 24"x18"x3" thick fell striking EE on the back of the neck and left hand. EE received two stitches in his left hand.
EE was loading rock-duster with dust. EE strained hemorrhoids out of rectum.
There has been an unplanned roof fall located in the No.1 entry 1 crosscut out-by survey station No. 3762. This fall does not impede travel and there has not been any disturbance in the mine ventilation. This fall has already been reported by phone to the MSHA hotline and a roof control inspector has been at the mine to investigate the fall.
