Employee was moving a motor by hand on a trailer and pulled a something popped in right forearm. EE was diagnosed with a strained muscle. EE started missing work on 5/15/2017.

The miner was pinning in #3 entry on the left side. Was pinning in a low spot and was on knees. After drilling hole, lowered the boom and the starter steel stuck in the roof. Put hand on the boom to pull self to a standing position when the steel came out smashing right middle finger against the pot.
Employee was getting out of coal hauler and twisted left ankle. Started missing work on 3/5/2016.

A roof fall occurred on the S2 M4 right side return air course measuring approximately 17' w x 17' l x 12' h.
Employee was pulling a stuck drill steel from the roof and strained his shoulder.
Employee was operating a shuttle car, and was getting a load at the miner. The car got on top of the bumper of the miner and was lifted a few inches. When the car pulled away, it dropped and caused the operator to bounce and jam his head into the canopy.
Placing drill steel to top, and steel popped back smacking thumb resulting in stiches.
Employee was exiting out of vehicle by office to turn in paper work. He cannot remember if he missed the last step on vehicle but knows he stepped in a hole and went rolling on ground.
