Showing all 10Bolter operator drilling roof. When adding pusher steel, stumbled / slipped and put uncontrolled feed pressure up while right hand was still around the steels EE was marrying. This caused the steels to bend toward the mast, pinching EE's right ring finger between the steels and the feed cylinder. This caused a skin avulsion roughly 3/4" x 3/4" at the tip of the finger.
Employee was carrying 3 drill steels bound together, as they walked through a known slick area, they transitioned between solid floor and mud which caused them to slip and fall with steels in hand, they reached back with right hand to break their fall, crushing right pinky finger.
Tasked with pre-use inspection of a high voltage switch gear. Injured noticed dust on the receptacle and went to wipe with a rag. A capacitive discharge entered EE's hand and body. This resulted in a small burn to EE's left and where the discharge exited and muscle soreness in forearms, shoulder, back.
Employee was cleaning up a belt splice area and employee was lowering the rollout of the belt core. Hook came off end and the core fell which caused injury to their pinky finger
While building a Kennedy overcast, injured was kneeling on the solid, left for overcast crossing, over a 1-2' gap above the completed overcast wall below. The injured lost balance and slid between the gap. Injured stuck left arm out to brace against impact, catching the Kennedy decking before sliding approximately 6' between the wall and solid ground below.
Unloading Richwood impact clusters off the cookie sheet of an M2 shield hauler. Unloaded two cluster without incident where one man would push and the other would pull. The injured pulled the third cluster into a stationary cluster, smashing EE's finger between the two. EE sustained a small fracture in the tip of EE's right middle finger. The wound required 8 sutures to close.
EE was hanging line brattice in xc20 between #4 and #5 entry with a insulated hook and when EE lowered EE's hand the end of the hook had caught EE's glove tearing into the back of EE's left hand, between EE's index finger and thumb
Employee was operating the left side of roof bolter, when the canopy got hung up in the roof mesh, they tried lowering the canopy and it would not come down, then they tried lowering the whole boom and it broke loose abruptly striking their hard hat/head against the canopy.
Crew was walking out of the mine. Employee, without warning, fell to the ground. EE was immediately checked, and no pulse was found. CPR was initiated without success. Internal investigation has determined that this was a personal medical condition.
