Employee claims to have strained left shoulder while pulling rock dust hose through a mandoor. EE completed shift and asked to complete an incident report as a precaution. Employee called off work on Monday, November 6, 2017 citing the "injury".

Employee claims that back spasms started when employee bent down to pick up a roof bolter cable plug. Employee was transported to a local hospital, examined, and released. Has not returned to work since.

Employee was riding out of the mine on a section portal bus. As the bus traveled through a switch, the bus was jarred from side to side. The jarring motion allegedly caused a sprain/strain to the employee's lower left back. Employee completed shift and went to a doctor later that day. Employee has not returned to work.

Employee was stacking cement blocks on a pallet when ee felt back give out and ee dropped a block on upper left thigh. Employee was transported to a local hospital where all tests were negative and ee was released. Employee has not returned to work.

Employee was along the track when a bus inby ee was entering a spur. The bus snagged the lifeline, stretching it tight, and breaking it. The lifeline snapped back toward employee and a directional cone hit ee on the cap light. Employee continued working but started feeling tingling in face and decided to seek medical aid. EE was transported to a local hospital.

Employee was attempting to lower shield # 118. The shield failed to function from adjacent controls. Employee used the in-shield control to lower the shield. When EE did this, the shield unexpectedly moved forward and shoved EE to the mine pavement.

Employee used their foot to attempt to throw a switch. They slipped and fell backward onto their SCSR unit. This caused pain to their lower back. Transported to local hospital. Negative x-rays, etc. Released to return to work but did not report in on the next scheduled shift.

Unplanned roof fall on the 12 Right Longwall tailgate. Top "potted" around roof bolts at the tail of the longwall face. Set additional roof props, tightened existing cribs and continue to mine through the area.

While spotting belt rollers on the 13 Right beltline, employee bent down to pick up a top roller. EE felt a "pop" in the right knee and fell to the ground. EE was unable to put any weight on the leg and was brought out of the mine. EE was transported to a local hospital and diagnosed with a torn meniscus in the knee.

Employee was pulling a supply trip around the loop. The trolley pole came off the wire in a low spot. As the employee reached for the pole, apparently touched the metal portion of the harp & received an electrical shock. Employee completed shift. Experienced discomfort in arms later in the day and visited doctor. Employee states has difficulty raising arms.

Injured employee was removing a vent tube on the 13 Right section when a piece of rib rolled off and struck employee's lower left leg and foot resulting in a bruise.

An unintentional roof fall was discovered around 6:50 AM on the mainline track haulage near the "A" Shaft Bottom area. The fall was approximately 16' wide by 30 - 40' long by 7' high.

Employee was installing a post when they felt a pain in the elbow. Finished the shift and visited the doctor the next day. Worked restricted duty until 08-23-2017 when they called in saying the doctor was taking them off work completely.

Unplanned roof fall in the #2 heading at #9 block on the 12 Right Longwall section. Approximately 16' wide x 16' long x unknown height. Also projected into crosscut 2-1 for a short distance. No injuries or equipment damaged. Dangered and timbered off.

Unplanned roof fall on Empty Track near rotary dump. Approximate dimensions are 16'wide x 7' high x 30' long. No injuries. Fall was discovered during a pre-shift examination.

During daily inspection, a malfunction was discovered in the North Elevator. Originally thought to be an electric motor failure. The actual failure was in an electrical relay. The elevator was up and running around noon on 07/03/2017.

Employee was using portable drill to drill holes in the mine roof to install trolley wire hangers. The drill dropped suddenly, striking the employee's right elbow. They finished the shift and filled out an incident report when they left the mine. Went to a doctor and was placed on restricted duty. Worked 2 days and then saw another doctor who placed them off work.

Employee was assisting the loading of a belt tub onto a well bottom car. The tub needed re-positioned. Employee motioned the scoop operator to "slew" the tub over. The operator shoved the tub. Employee's foot was caught between the anchor point of the car and the tub. First aid was administered. Employee was transported via ambulance to a local hospital.

Employee was unloading a tub of assorted belt structure. As ee reached for a piece of structure, a belt rail slipped down from the stack and pinched the tip of right index finger.

As the employee stepped down from employee's bull dozer, employee felt a sharp pain in employee's right knee. Employee had difficulty straightening the knee. Employee completed employee's shift but called off on employee's next scheduled shift.

The employee was assigned to work near the head gate of the 12 Right longwall. The employee was recovering a joint of rock dust hose by cutting it off with a razor knife. The employee was holding the hose with left hand and cutting it with right and the blade lacerated left forearm. The employee finished the shift. Later had 3 stitches placed in the arm.

While walking by a portal bus, the employee caught the safety hook of the bus with EE's left foot. EE fell and twisted EE's ankle. EE finished the shift and went to EE's doctor later in the day due to ankle pain. X rays revealed a fractured ankle. Employee was scheduled to see a specialist on 5/18/17 and has not returned to work as of today.

On 4-15-17 the longwall headgate became impassable due the top setting down on the longwall stageloader. By 4-17-17 the wall had "pushed" enough of the material away to expose the strata that showed the top had fallen higher than the roof bolt anchorage zone making this a fall by definition.

As the employee attempted to pull a piece of trolley wire from under a track rail, EE felt a pain in back and hands. Employee finished shift and reported the incident as EE exited the mine site. EE reported off on next scheduled shift.

The employee was installing track rail on the 13 Right section. As employee pried a rail into place with a slate bar, they felt a sharp pain in the lower back. Employee completed the shift but called off on the next scheduled shift.

While roofbolting with a single head bolter a return hydraulic hose failed and sprayed hydraulic oil into the employee's left eye. Employee was transported to a local hospital where the eye was cleaned and medical drops were applied. Employee returned to work on the next scheduled shift.

Employee was examining the North Mains left return as part of a weekly exam. Employee jammed their head on a cable roof bolt near 150 block. Employee completed the shift and continued working regular schedule until March 28, 2017 when they called off due to neck pain.

Employee struck their head against a roof bolt while dragging cribs into old works. Incident caused tingling sensation in employee's neck. Employee was transported to a local hospital, treated, and released. Reported off the next day.

While placing a crib block under a section of panline, the employee caught EE's middle finger of EE's left hand between the crib and the section of pan. Employee completed EE's shift. EE went to the doctor the next day and was diagnosed with a fracture. EE continues to work alternate duty.

While watching a fellow employee troubleshoot a scoop electrical problem, an arc flash occurred. This caused a flash burn to the employee's eyes. Employee was brought outside and transported to the hospital. Was treated and released. Given antibiotic ointment by eye doctor.

Employee was working on a battery scoop. EE disconnected one lead of the dual lead batteries. As ee disassembled the battery plug, ee apparently made contact with both leads. This caused an arc flash that caused burns on hand. Employee was transported to the hospital where ee was treated and released.

Employee was assisting on the longwall move. Employee attempted to lift a 4" I beam. EE felt a "pop" in left groin. Employee finished shift and worked the next day. EE felt increasing soreness in the affected area and went to the doctor. EE was diagnosed with a hernia.

Employee was observing motormen attempting to put a derailed car back on track with inflatable lift bags. While inflating the bags, the middle bag in a stack of 3, flew out and struck the employee. Employee was knocked backward and landed on backside on the mine rail. Employee finished shift and worked the next day. Went to the doctor on the second day.

At 10:30 pm on October 23rd, a roof fall occurred at the 1 North sump. The fall measured 60 feet long, entry width wide, and 8 to 10 feet high. The fall did not impede travel of miners or disrupt ventilation.

Employee was pumping "glue" into the mine roof near the 11 Rt longwall face in the belt entry. As EE pumped the glue into the roof, a slab of rock/headcoal about 3'x4'x1' thick fell out between existing roof straps. The falling rock struck EE across back. EE stated that EE felt a pulling sensation in back. EE was brought out and transported to the hospital.

Employee was walking along the longwall stageloader when they stepped on a hydraulic hose valve. Their foot apparently activated the valve & the loose end of the hose abruptly snapped back & struck them in the face. Employee finished their shift. The employee visited a hospital emergency room the next day. Employee stated that the medication they were placed on made them ill.

Employee was installing supplemental roof bolts with a hand held roof drill. The drill steel caught, causing the drill to twist in grip. The twisting action caused a strain/sprain injury to right hand and arm. The employee worked shift on 10/05/16 but started missing on 10/06/16.

Employee was cutting a cable bolt out over the longwall headgate to gain clearance for the stageloader to advance. When EE completed cutting the bolt off, a rock about 18" x 18" x 3-4" thick dropped out of the roof. The rock slid down previously installed roof screen and struck EE on the right foot.

Employee was adjusting a sensor on the belt. EE was sitting on the bottom rail of the belt guard. The rail broke causing EE to fall backwards. EE put left hand out to catch self resulting in an injury to left thumb. Employee finished the shift and has continued to work since. On the 21st of September, x-rays revealed a fracture to the thumb.

Employee stated that while walking along the longwall face, employee tripped on a relay bar. Employee attempted to break fall by reaching with right arm. Impact caused a strain to shoulder.

Employee was attempting to remove a piece of belting from underneath a belt tail roller with a set of come-a-longs. As EE tightened the come-a-long, the belt dog slipped. The sudden release of tension caused right arm to move suddenly. EE reports feeling a "pull" in the shoulder.
