The Contrary Service Hoist had a roll of conveyor belt loaded on it that was attached to the 30 Ton Overhead electric Hoist that is used to unload the belt. The PC and Overhead hoist remote control had electrical problems and this interfered with the use of the Elevator.

Injured was operating the tail side shearer while they were cutting out on the head. A rock came off of the tail drum and struck EE on the right thumb causing a laceration resulting in 9 sutures.

The employee was starting to drill the first hole in the last row when a triangle shaped piece of rock measuring 12.5" x 6" x 1.5" fell from the roof around the drill steel deflecting off of the rotating drill pot striking the employee on the right wrist just above the glove line causing a laceration that required sutures to close.

The injured was trying to insert/align a shear pin in the relay bar on shield #2. While trying to align the pin, a come-along that was hooked from the header to the relay bar became unhooked, striking the employee on the top of the right hand causing a laceration which required sutures to close.

The injured was walking in 5 north bleeders from 17 east to 16 east. Their left foot slipped while walking in wet and muddy rock dust and when they went to catch themselves with their right leg, they felt a pain in their right knee.

Shear Operator was spotting a shield to catch rock on the LW face, a piece of rock rolled out into the panline and hit the spill board and a piece of rock measuring 54"x14"x3-8" broke off after striking the spill board and fell into the walkway and struck the the employee on the right side of right foot causing a laceration which required 2 staples to close.

While attempting to accessing the offside area of the 2 East belt, to pre-shift the drive motors, the employee moved under the conveyor structure/Cross under guard. During this movement, a portion of the employee's coat contacted a bottom conveyor roller. Resulting in abrasions and swelling to right elbow and abrasions to right arm.

The employee was at 7 East Brk 60 at the Double doors picking up trash/old pipe and loading it on flat cars and as they threw the pipe/trash on the car, employee states that they lost balance and caught themselves with left hand and used the flat car to catch themselves, they felt a pop in left shoulder.

The angle iron for the stop switch broke off and got fouled between the elevator and the beam causing damage to control leads. The control leads were replaced and PLC's had to be reprogrammed.

A Maintenance Foreman and a passenger was in the process of traveling from 9 Vent bottom to 17 East section to work on high voltage power line. While in the second block light of the 5 North back track, at crosscut #44 it is believed that the injured employee's cap light struck a low spot on the mine roof (57 inches) causing trauma to their neck and vertebrae.

Employee lifting an 18' ventilation slider to line up to slide into a piece of 20' ventilation tubing, an employee in the back of the slider heard someone say push the slider and when EE did the foreman's finger was struck against the D-ring of the slider and the 20' tubing. Resulting in a fracture, and a laceration that required 2 sutures to the tip of EE's left middle finger.

The injured inadvertently placed a hand on the bolt tray to attempt to move, when the rock fell striking them on the left hand that was on the bolt tray, causing a laceration which required 3 sutures to close.

Resulting in a laceration and fracture to the tip of EE's finger.

The injured was attempting to cut a piece of mine belt to be used as skirting material. While using both hands on the utility knife, EE lost EE's balance and slipped, and while EE's arm was stretched out to catch self, EE's left wrist contacted the knife causing a laceration, which required 3 sutures to close.

The injured was trying to remove a broken piece of conveyor flight on the Stage Loader tail sprocket inside the NDRE box and as EE was prying with a pry bar, the bar slipped causing EE to strike EE's right hand against the broken piece of flight. This resulted in a laceration to the ring finger knuckle which required four (4) sutures to close.
