EE strained groin area while loading heavy items into a truck on 10/26/2025. EE reported injury but refused to see doctor and continued working until 11/9/2025. Examined by doctor on 11/10/2025. Diagnosed with a Hernia on the right side

EE burned the palm of EE's left hand when EE unknowingly placed it on the surface of a hot metal work bench. Another EE had been working metal, with torches, on the bench a few minutes before the injured EE walked up to talk to EE. EE was taken to Clinic and examined & treated for 2nd degree burns then released to return to work.

EE was using a battery powered hand grinder to cut metal pins. The cutting wheel stuck on the metal, causing the grinder to kick-back, coming in contact with the EE's left thumb. EE sustained a laceration that required 6 stitches. EE received a work release, giving time for the laceration to heal.

10/2/25 EE working on surface when a Front-End Loader ran over a wooden pole. The pole swung knocking EE to the ground. EE hit EE's head & R-shoulder on ground. EE reported incident but continued to work EE's normal shifts until 10/6/25, then requested to have right shoulder checked. Doctor excused EE from work until cleared by MRI.

9/30/25 while walking up steps EE tripped and reached out to catch self, but missed and fell, striking EE's face and left shoulder on the handrail and steps. EE continued to work EE's normal shifts until 10/7/25, when EE requested to see a doctor to have EE's left shoulder checked. Doctor excused EE from work to do Physical Therapy before returning to work.

While unclogging drill steel wrench by hitting it on the roof bolter frame, the EE swung the wrench, and the back of EE's right hand made contact with a roof bolt plate located on the supply tray. EE sustained a laceration to the back of the right hand. EE was taken to the Clinic and received stitches to close the laceration. EE was released to return to work.

Roof Bolting, EE was bending bolt to get it in roof. While pulling back on the bolt, they felt a pop in their right wrist. EE did not want to see a doctor and continued to work. Six days later - went to doctor. Diagnosed with Wrist Tenosynovitis and was released to return to work. After working 2 more days, EE went back to doctor (5/8/2025). EE is off work waiting for an MRI test.

EE was in the surface fabrication shop. EE was holding a ¾" pipe as a punch, to push a keeper pin into place. Another EE used a pipe, as a hammer, to hit the pipe EE was holding. When the pipes made contact, the pipe being swung ricocheted and struck the small finger of the EE's left hand. This caused a laceration that required stitches, and a fracture to the finger.

EE was in the surface shop pulling a rolling tool cart behind EE. The cart caught the heel of EE's left foot, causing the cart to run up the back of EE's heel and put weight and pressure on EE's foot. X-Rays showed a fracture to the big toe's metatarsal bone of EE's left foot.

EE on ladder to foam top of Vent Line Curtain. Ladder moved & EE fell to mine floor. EE landed on feet, but left leg folded under EE upon impact. Doctor referred EE for MRI to diagnose if any damage to ACL. MRI was not available till 2/27/2025 and results not received till 3/6/2025. MRI was normal but was diagnosed with knee sprain and referred for PT.

EE was using a battery hand grinder. The tool slipped and when grabbing, EE hit the trigger. This caused the grinder to flip around. The spinning grinding wheel cut the back of EE's left hand, causing a laceration that required 9 stitches. It also chipped a finger bone & ligament. EE was referred to an Orthopedic Doctor.

After the EE finished the shift, EE was walking from the bathhouse to EE's car. EE stepped wrong and twisted their right ankle. EE thought was fine and went home. When EE arrived at home, EE's foot was hurting. EE was taken to the doctor and examined. An X-Ray showed a fracture to the 5th Metatarsal (small toe).

A Roof Fall occurred immediately to the left of the Lift that the Continuous Miner was in, and just outby the Mobile Roof Support units. In the #3 Entry of the 4th West Pillar Section (just inby XC #19). It was not above the anchorage zone, and only about 4 feet high, 10 feet long and 14 feet wide. No one was hurt or entrapped and no ventilation was damaged.

On 10/24/2024 an EE was hand loading Roof Mats, by self, and felt a pop and pain in EE's right biceps muscle. EE was examined at the Clinic and an MRI was ordered for diagnosis. An MRI was not available until 10/30/2024. EE was diagnosed, after the MRI, as having torn tendons/muscles in EE's right biceps.

EE used 16-inch screw bar to hang Ventilation curtain. The screw bar slipped out of EE's hand and fell, striking EE above the left eye, causing a small laceration. No treatment needed. Finished shift and went home. Next day EE did not feel good. Went to the ER and was Diagnosed with a Concussion. Doctor prescribed days off.

A Rock Slab from the Surface Highwall slid down and covered the #1 and #2 Portal Openings. The size of the Rock Slab that slid down was approximately 3 to 5 feet thick, 180 feet long and 30 feet high. All miners were evacuated via the #3 - Right Intake Portal. All miners were accounted for and no injuries were reported.

Unplanned Roof Fall - Outby Intake entry of South Mains (Entry 5 between XC #6.75 and XC #7). It occurred sometime between 2 & 2:10 PM. It is about 3 feet high, 20 feet long and rib to rib, in the Primary Escapeway. No one was hurt or entrapped and no ventilation was damaged.

While helping change out a pump motor, EE was moving the motor into place and got EE's left ring finger caught between the motor and the frame. EE sustained a laceration to EE's left ring finger that required 8 stitches. EE was treated and released to return to work.

EE was unhooking a trailer from EE's machine. EE was throwing a wood crib block under the front of the trailer to prop the trailer up. When EE threw the crib EE smashed right index finger between the crib and the trailer. Sustained a laceration the required 5 stitches and a fracture to the distal joint.

EE injured while servicing, changing oil, on Co #308 / DST 07 LHD Loader. EE was reinstalling the belly pan when it slipped and fell, smashing EE's left ring finger between the belly pan and the ground. EE sustained a laceration and Distal fracture to the finger. EE received stitches, finger splint and antibiotic.

An EE was injured when EE stepped off the Roof Bolter Operator Platform, with the platform raised about 2 feet off the mine floor. EE reached up to the bolt tray to retrieve the Drill-Steel and it slipped off the tray and struck EE in the face, causing a laceration to the left side of lip/cheek. It required 2-stiches to close the laceration.

Roof Fall - 3rd West Retreat Mining Section (Entry 5 inby the Pillar Line) between XC #7 & #8. About 15' wide by 15'long and 3' high. NOT at or above the anchorage zone, fell on top of a battery scoop. One miner was entrapment in the cab of the scoop. The miner escaped with no injuries. Preliminary diagnoses; Strata failure due to weight over-ride from retreat mining in Entry 4.

EE injured while removing a hose from a Fletcher MRS leg cylinder ram. EE inadvertently hit a fitting on the check valve, next to the hose EE was removing, with a wrench. The fitting failed and released high pressure oil that hit the EE's right hand, causing a laceration and injection of oil. A surgical procedure was required to clean the oil out.

EE injured operating continuous miner. EE split the tracks to turn the CM to the left and was struck on the left hip by the CM Tail Boom and then caught between it and the mine Rib. EE sustained contusions to both hips, no evidence of broken bones. Later, during EE's stay in hospital, EE noticed pain in right hand. X-Rays show a possible small bone fragment at a metacarpal base.

Climbing into the back of a lube truck to get a 5-gallon can of oil. Placed foot on ball of the hitch and pulled myself up. My foot slipped off the ball and I fell backwards onto the ground, landing on the forks of the telehandler. Right arm went behind me as I fell and I landed on it.

An unplanned Roof Fall occurred at an unknown time. Right Return Air Course of Main West, Entry #6 XC 21. Approximately 5 ft. wide by 6 ft. long and 4.5 ft high. It is NOT at or above the anchorage, but has impeded travel. No injuries, entrapment and no ventilation interruption. Preliminary diagnoses; Strata failure due to deterioration of the seam transition area.

EE was injured when lifting a bottom belt roller up onto their shoulder. They felt a pop and pain in their chest and left shoulder. EE has a pre-existing injury that is non-work related. EE was diagnosed to have strained their Sternum and Left Shoulder. EE was given a Modified Work Release.

EE was injured after exiting a Skid Steer. They jumped out of the cab instead of using 3 pints of contact. When they landed on the mine floor, they twisted their right ankle. X-rays were negative. Diagnosis is sprain/strain to the Right Ankle. EE's ankle was wrapped and they were put on crutches and given modified duty until follow-up appointment.

An EE was injured when they stepped back on the Roof Bolter Operator Platform, and missed footing. They fell between the platform and the TRS Scissor-lift frame. They struck the left side of their face on the frame, causing a laceration to the left side of their face. It required 3-stiches to close the laceration.

EE was injured when descending a ladder. They misplaced their right foot and only caught the ladder rung with the tip of their boot. This caused their ankle to hyperextend. They felt pain in their right foot and knee. X-rays were negative. Diagnosed with sprain and given 3-days off work. After the 3-days EE still having pain in knee and has an appointment to see doctor.

EE became ill, nauseas and light headed, while operating a diesel powered scoop in the face. EE finished the shift. The next day EE returned and worked EE's shift, stating EE still felt nauseas. 2-days after incident, before coming to work, EE went to doctor and was diagnosed with CO poisoning. The EE self-terminated EE's employment the same day.

An employee was injured while shoveling accumulations in the belt entry. EE used the shovel to knock accumulations off the structure. EE's shovel caught a roller and flipped the shovel back at him. The shovel struck EE on the chin, causing a laceration. It required stitches to close the laceration.

EE was injured while tending Shuttle Car Cable. EE bent over to grab the cable, at the same time the operator started the machine. The Hydraulic power to the cable reel was on, causing the cable to pull tight. This caused the cable to come up off the mine floor and the cable struck the EE on the chin. The EE sustained a laceration under the lower lip that required stitches.

An unplanned Roof Fall was found at 6:50 AM in the common Intake of 3rd West. In Entry #3 at XC #35.5. It is 35 ft long by 20 ft wide and 5 ft high. It is at the anchorage zone and impedes travel. No one was injured or entrapped. Ventilation was not interrupted. Preliminary diagnoses; strata failure due to roof support not having good anchorage in competent top.

An EE was injured while cutting through plastic with a razor knife. The knife slipped out of control and EE cut left leg, just above the knee. It required 10 stitches to close the laceration.

An EE was injured when EE slipped and fell while walking down an overcast ramp. EE twisted left ankle. EE was diagnosed to have sustained soft tissue damage to the left ankle, requiring modified duty and the use of crutches.

An EE was injured while removing a keeper pin out of a track pad on a continuous miner. While hitting the keeper with a hammer the hammer slipped and smashed EE's left thumb, between the hammer and the track pad. It required stitches to close the laceration. X-rays also show that the thumb has a small fracture, requiring modified duty.

No one was injured or entrapped. Ventilation was not interrupted. Preliminary diagnoses; strata failure due to seam transition area.

An unplanned Roof Fall was found in the Intake Air Course of 3rd North, Intersection of Entry #9 & XC #7. It is approximately 20' wide by 20' long and 7' high. No one was injured or entrapped. Ventilation was interrupted. Preliminary diagnoses; Strata failure due to water saturating the upper mudstone band, causing it to swell and weakening the Roof Control.

An EE was injured while moving a water pump. They slipped on the muddy mine floor and struck their jaw on top of the pump. Diagnosis was a fractured tooth that required dental work.

A water inundation occurred due to a Flash Flood from a rainstorm. The Flash Flood washed out a dam in the drainage system, on the surface, which allowed water to enter the mine. The water created holes in four stoppings, one Overcast and one Box Check. No major air changes occurred and no one was injured or entrapped.

An unplanned Roof Fall occurred at 3:45 AM in the Intake Air Course of Main West, Entry #2 XC 21. The cave is approximately 18 ft. wide by 10 ft. long and 6 ft high. No one was injured or entrapped and no ventilation was interrupted. Preliminary diagnoses; temperature change caused air-slack and Strata failure around supports.

An EE was injured while lifting a 5 gal can of oil onto the bed of a pick-up. EE felt a stretch/rip and a bump next to EE's belly-button. The Hospital ER made an appointment for the EE to see a general surgeon. Nine days later the general surgeon diagnosed the EE with a Hernia. Surgery was scheduled 2 weeks after the appointment with the surgeon.

An EE was injured while operating a battery scoop. The service brake did not work properly. EE panicked and hit the E-stop, killing all the power and controls. The scoop rolled forward and hit the rib. EE hit head on the cab, sustaining a neck strain.

An EE was injured when attempting to hang continuous miner cable to the mine roof. The hook EE was using to attach the cable to the roof screen slipped and the cable fell down. The EE was holding onto a drill steel EE was using to hang the cable when it fell. The jerking motion injured the EE's right shoulder. Diagnosis was a Sprain/Strain.

An EE was injured installing a rear axle in a Mantrip. With a floor jack holding one side & a ratchet strap holding the other side, about 2 feet in the air. EE attempted to lower it with the ratchet strap and it released and fell, the axle struck EE on the right lower leg. Diagnosis was a contusion.

An employee was injured while installing/working on the communication system. The employee tripped and fell face first into the mine rib. Employee's hard hat hit the rib and slid down striking employee's glasses and nose. Diagnosis was a laceration and fracture to the nose.

An employee was injured while attempting to replace a heavy mechanical part on a continuous miner. When attempting to move it into place, the part sild and fell, striking right ankle. Diagnosis was a right ankle sprain / contusion.
