Showing all 9An employee was attempting to place a plate in the pioneer shaker table when EE received a laceration to EE's right hand. EE was utilizing a sledgehammer when the hammer slipped and caused EE's right knuckle to abrade on the pioneer shaker plate.
On Monday, 01/12/2026, Souter Mechanic, was attempting to weld on machinery and received a slag burn to the inside of their ear. Mechanic had the welding torch positioned below them, and when welding the metal popped and a splash went into their left ear.
As EE was reassembling the silo pipes, EE attempted to use an impact tool and wrench combination to tighten a bolt. The impact torqued the wrench, and EE's little finger was pinned between the wrench and the pipe.
EE had opened the haul truck hood with a crowbar, and then EE was attempting to use the bar to prop the hood open while EE was filling the truck with antifreeze. In the process, EE shifted position and the crowbar came loose and crushed EE's hand between the hood and the engine bay.
Employee was on the lower platform of the control house, assisting in freeing a rock obstruction from the mouth of the jaw crusher. Employee had opened the top of the crusher cage so that the operator could manually free the obstruction. After obstructions were freed, the rock entered the jaw and the vibration caused the guard to fall and impact employee's left hand.
The employee was climbing down the ladder of a haul truck, missed the last few steps and fell to the ground.
The miner was walking toward the plant and noticed the front end loader coming up behind EE and stepped to the left side of the road to move out of the way. As EE stepped down, EE's left ankle rolled causing an ankle injury.
Operator was attempting to repair a leaking seal on a rotary valve. The operator placed their hand between the chain & the drive sprocket. EE's hand was pulled into the sprocket & crushed between the chain & the sprocket. The incident resulted in a broken thumb & 2 broken fingers in addition to a laceration across EE's entire hand.
