Employee was dismounting the front-end loader using the stairs on the rear of the machine when EE fell and slid down the machine's stairs. The employee continued to work until EE sought medical evaluation on 4-28-2026 and was removed by a physician.

The employee was released by physician and returned to work on 6/9/25 from a previous injury. On 6/11/25 the employee was taken back out of work by physician when they reported continued back pain from the pre-existing injury which was preventing them from conducting normal mine work.

Employee strained back at the start of employee's shift as employee was pulling themself up to the cab of the excavator.

Employee developed welders flash (Photokeratitis) to their eyes when welding.

Employee slipped and fell hitting their elbow and arm resulting a sprain/sprain on 1/15/25. The employee continued to work normally until they were later taken out of work by a physician on 1/30/25.

Employee was exiting the mine property in personal vehicle at the end of shift when employee collided with an empty coal truck that was traveling in the opposite direction on the mine haul road.

On 10/12/24 employee was outside the water truck and was in the process of entering the truck and twisted knee. Employee continued to work until employee was taken out of work by a physician on 10/23/24.

On 9/25/24 employee was lifting a 55-pound bag of flobeads (Dry Cat) when EE felt a sharp pain in their back. Employee continued to work until EE was taken out of work by a physician on 10/22/24.

A coal rib sloughed out striking the employee in the lower back.

The employee was lifting a five gallon can of oil off the supply car and felt a sharp pain in right shoulder.

Employee was conducting repairs on the bird cage/agitator section of the centrifuge when the bird cage rotated and pinched the employee's fingers resulting in lacerations. The employee received sutures to 2 fingers as a result of the injury.

The employee slipped when dismounting the equipment and got EE's left pinky finger and ring finger caught between a lid, resulting in a laceration and strain to EE's fingers.

EE was instructed that an exposed electrical cable was inactive. During the work operation (pumping out overflow rain water) EE contacted cable and received a shock. EE was taken to local ER, checked out and released.

The continuous mining machine inadvertently intersected a gas well casing.

Employee stated that EE was tramming the D9 dozer when the park brake inadvertently became engaged causing EE to jolt EE's back. The employee completed shift and as EE stepped onto the mantrip, EE stated EE felt pain in lower back. The employee was taken to the ER and determined EE had a strain. The machine was inspected, and no deficiencies were found.

The employee was installing a roof bolt and the bolt came out of the drill chuck near the top. As the employee attempted to get the bolt back into the drill wrench, employee got a finger caught between the wrench resulting in a laceration.

Employee was using a handheld chain ratchet set to hold one side of a pre-wet shaker screen as EE was changing a feed end spring on the shaker. In the process of changing the spring, the hook broke on the ratchet set resulting in the employee's hand to get caught between the spring.

Employee was drilling a hole in the mine roof to install a roof bolt and as employee was retrieving the drill steel out of the hole employee placed hand on the drill steel as it was spinning.

While breaking down a feeder for belt move, injured employee caught left thumb between crib block and roof bolt, resulting in laceration. Injury was not due to a training or compliance issue.

An unplanned roof fall was located in the primary intake escapeway 5 breaks inby the fan at spad No. 1130. This was in the #2 entry and measured approx. 20 ft. long, 16 ft. wide and 6 ft. thick.

Employee was using a metal pipe to dislodge material from a sweeper truck and caught middle left finger between pipe and a fender on truck.
