EE was changing bucket teeth on an excavator. When EE hit a tooth with a sledge hammer, a piece of the hammer head broke off and penetrated EE's left inner thigh. EE was taken to Palo Pinto General Hospital for medical care.

Showing all 4Operator was preparing to dump scrap brick, the dump gate latch stuck and would not open. Operator exited the cab and manually opened the latch, while walking back to enter the cab, EE tripped and fell, striking EE's right arm on a broken brick causing a laceration just below the elbow.
Operator of Tandem dump truck was driving a loaded truck on the haul road to the dump area. Driver fell asleep and veered off the road contacting the berm on the side. Driver attempted to correct the truck and caused truck to over turn onto the roadway. Driver sustained lacerations to the right hand and left elbow. Driver returned to work 7-03-19.
Prior to start of shift on 4/5/17, employee reported a pain in their neck & was sent for medical evaluation. Employee operated a bulldozer the prior day to spread clay on a stockpile. The employee stated that no incident or accident occurred on 4/4/17 before noticing this pain. Employee later described turning their head to back the dozer as a contributing factor.
