Employee was mining underground with a pneumatic chipping hammer. The chipping hammer glanced off a rock causing the moil to puncture the employees right foot. Employee was treated with basic first aid on site and then transported to medical care.

Showing all 5The genset broke down on 01/23/2025. The hoist was inoperable while we were waiting for parts and a repair person. MSHA inspector arrived on site on 1/27/2025- citation was issued per above conditions. Table Rock Minerals was unaware that MSHA needed immediate notification. Genset was repaired on 1/29/2025- hoist is now operable.
WHILE UNLOADING TIMBERS- EMPLOYEE INJURED RIGHT KNEE
While manually moving pieces of rock, the miner pinched the tip of EE's left index finger between two rocks. EE lacerated the fingertip requiring stitches.
THE MINER PINCHED SKIN ON THE PALM OF RIGHT HAND AGAINST THE EDGE OF THE SLIDE PIPE. THE MINER CUT THE SKIN ON THE PALM OF THE RIGHT HAND WHICH REQUIRED STITCHES.
