Employee unhooked lanyard and fell through roof resulting in fatal injuries.

Employee changed a bearing on an exhaust fan and was reinstalling the V-belts by hand when left pointer fingertip was caught between the V-belt and pulley sheave. The belt was rotated to free their finger. After removing their glove, a laceration was found. Employee was taken to the hospital, received four sutures, and returned to duty after treatment.

While monitoring a slurry tank pump due to abnormal noise and visible smoke, the employee was positioned in the area when material was unexpectedly expelled from the pump. The employee stepped backward and lost balance, resulting in contact with the ground. The employee reported an abrasion above the right eye and discomfort to the right hip.

Drilling hole in rubber gasket which slipped out and struck employee's hand causing laceration on right thumb.

An employee was using a break-over binder and chain to secure poly pipe to a float, the chain slipped, causing employee to fall awkwardly, (same Level) breaking EE's lower left leg.

The operator was starting a gas-powered air compressor by pulling the pull rope. On the third pull, the operator felt something pop or pull in their left shoulder. The doctor ordered the operator off work for 4 days and then EE can return to normal duties. EE was also advised to follow up with their primary care physician.

Operator was using a 2 pound sledgehammer to remove a worn tooth from an excavator bucket. When the operator hit the worn tooth (one time) with the hammer, a metal shard broke off of the worn tooth and passed through the operator's pants and the top of EE's leather boots before penetrating the operator's left leg, just above EE's ankle.

Miner struck by 'clay boulder' when EE crawled into a feed hopper to clear an obstruction. The miner suffered muscle damage requiring hospitalization and medication. Diagnosed with Rhabdomyolysis. No broken bones were found during the examination.

Miner suffered 3rd degree burns to the abdomen when a hose popped off the seal pump in the process of removal. Burns required debridement and an allograft.

Employee was adjusting the trunnion bearings of the dryer drum. Employee in 102° weather started to feel hot/ill, notified supervisor. EE was then taken to a break room. First aid administered. Symptoms worsened, decided to take employee to the hospital. When employee was stood up, EE lost consciousness but regained it a moment later. Taken by ambulance for medical treatment.

While observing another miner removing teeth from an excavator bucket from approximately 5-6 feet away. The injured miner was struck in the hand by a shard of metal (approximately 4mm X5mm) that broke off of the excavator tooth.

Employee was in the control room preparing for shift change. Employee then noticed co-worker had a knife on the table and asked if EE could look at it. Co-worker handed the knife to the employee. EE ended up grabbing it wrongly by the blade, cutting self in the process. Laceration required 6 sutures to close the wound. Employee returned to duty after medical treatment.

Operator slipped from top step/deck of 980K loader, landing on their back on concrete deck of sand storage area. Suffered injury to their head.

EE was attempting to roll a 4" dia x 8ft steel shaft onto the forks of a forklift. The forks were jammed against the bottom rail of the trailer carrying the metal shaft. When the forks were tilted back, they sprung upward catching the EE's thumb between the top rail of the trailer and the shaft, causing laceration. Required 5 sutures.

Employee started shift on 8/28/21 at 6PM operating a Loader. At 2:30AM (8/29/21), they stopped Loader operation for a break. When they stepped down onto last step, they reported left knee bent backwards causing pain. Medical care diagnoses include: 1) sprain of unspecified site of left knee, 2-3) pain in and effusion unspecified knee, and 4) other bursitis.

Plant Maintenance Employee was attempting to align mounting points on exhaust duct work with a bull pin when EE's grasp on the bull pin slipped and EE was struck on the mouth/nose, lacerating EE's lip.

A contractor truck driver was opening a hatch on top of the trailer when the driver fell off of the truck. The driver was using a safety rack to access the hatch on top of the truck. The driver complained of back pain. 911 was called and driver was taken to the ER via ambulance. X-rays showed no broken bones. Driver was released with "bumps and bruises."

Conveyor belt shifted causing overloads to trip. Maintenance discovered the take ups had failed. They were trying to remove them to do repairs when the load shifted catching the miner's arm.
