Employee was mounting a piece of equip when EE said felt a stinging feeling in the shoulder. Reported to the foreman. Employee was seen by Dr on 8/27/20 and was advised EE must have a pinched nerve. Employee referral and evaluated 9/11/20. Dr advised EE had 3 herniated disc in the back & approved EE to continue WC. Treatment to date MRI.

Employee was getting metal out and tripped and fell on a belt roller. This resulted in a laceration requiring 11 stitches to EE's left leg.
