Employee was mounting a piece of equip when EE said felt a stinging feeling in the shoulder. Reported to the foreman. Employee was seen by Dr on 8/27/20 and was advised EE must have a pinched nerve. Employee referral and evaluated 9/11/20. Dr advised EE had 3 herniated disc in the back & approved EE to continue WC. Treatment to date MRI.

Was walking back to truck from taking piezometer readings of Dam#1 at Moss 3 facility. As crossing spillway, slipped on slick surface and slid 10' before culvert drain emptied into spillway carrying EE another approx. 150'. A large rock in spillway stopped slide and threw EE out of spillway. Rock hit him on tailbone and movement on right side below hip is stiff & sore.

Employee was climbing down from his loader and hurt his left ankle on last step. His ankle pain persisted but he continued to work until 12/05, when the Doctor placed him on work restrictions for two days.
Employee was hooking up a trailer and was cranking the lift jack when he strained his back and left shoulder.
Employee went into cab of locomotive being serviced to get pry bar, and fell in the cab where the floor board had been removed while repair work was being done on the locomotive. Employee continued to work his regular job until his scheduled surgery on 9-10-08.
Employee slipped on ice falling down on his right shoulder. He worked until he was taken off work due to rotator cuff surgery preformed on 4/10/09. He became lost time as of that date.

Employee was hooking a chain around a piece of pipe to lift a roll of belt. He threw chain up and over pipe. Chain came back striking employee. Went to doctor 11-9-07. Started losing time on 11-10-07.
Employee was lifting a 6' step ladder out of the mechanic's truck and felt pain and burning in lower back. Employee began losing time 3-25-08.
EE was using a piece of conduit to clean out a feeder in the silo-a piece of rock fell striking the conduit, catching the employee's hand between conduit & I-beam.

Employee was pushing on a piece of pipe trying to get bolts to line up with the coupling - felt a sharp pain in his upper right arm & shoulder. Went to doctor on 4-12-07. Started losing time on 4-13-07.
EE was walking from the High Head Building to the back porch, strained his right knee. Went to the Dr. on 2/19/07, had surgery on 2/20/07.
Injured was getting a shovel off the wall to get a sample. He got his feet tangled in angle iron that was laying on the floor causing him to fall striking his right knee & hand on the metal grating fracturing his right hand.
INJURED EMPLOYEE WAS WORKING ON A PIPE JOB. ANOTHER EMPLOYEE WAS GRINDING IN THE AREA NEXT TO HIM. HE STATED THAT HE FELT A STING IN HIS RIGHT EYE AT THE TIME BUT WAS UNAWARE OF ANY METAL HITTING HIS EYE. HE DID NOT REPORT THIS UNTIL 3/15/06. THE DR. HAD TO REMOVE THE METAL SLIVER FROM HIS EYE.
This employee has been diagnosed as having carpal tunnel syndrome in his hand - due to repetitive trauma. Corrective surgery was done on 2-13-06.

Injured was releasing a brake on a railroad car. He stated that he had to jerk on the T-handle hook to break the brake loose. When he jerked on the T-handle it came out of the brake wheel causing his right elbow to strike the drawhead of the railroad car.
Injured was dismounting a d-6 dozer when his foot slipped off of the pusharm and wedged between the push arm and the dozer frame. As he fell backward it twisted his left leg. He started loosing time 25 of 4-11-06.
Injured was moving a 55 gal. drum of oil with an overhead hoist. As he sat the barrel on the ground, the hook slipped and caught his little finger on his left hand between the hook and the barrel.

EMPLOYEE WAS WORKING ON A WATER PUMP IN THE PUMP STATION. HE JERKED HIS HAND BACK AWAY FROM PUMP, STRIKING HIS RIGHT THUMB AGAINST PUMP CASING.
Injured was reaching his co-worker a chain when his foot slipped off of the belt drive frame causing him to fall striking his right arm. Elbow fell against the belt drive roller.
EE WAS ADJUSTING A BRAKE TENSION BOLT WITH A 10"" ADJUSTABLE CRESCENT WRENCH. THE WRENCH SLIPPED, CAUSING HIM TO HIT THE TOP OF HIS HAND ON A NEARLY BOLT.
INJURED WAS PLACING A PIECE OF 1"" PLATE IN THE DRYER, THE PLATE WAS HANGING ON A CHAIN HOIST, WHEN THE WEIGHT SHIFTED ON THE PLATE, CAUSING HIM TO TWIST HIS BACK.
INJURED WAS PULLING ON A HYDRAULIC OIL FILL HOSE AND TRYING TO MOUNT THE DOZER AT THE SAME TIME. THE STEP ON THE DOZER'S PUSH ARM WAS WORN & HAD FROZEN REFUSE ON IT CAUSING HIM TO STRAIN HIS BACK. EE BEGAN LOSING TIME 3/12/04

INJURED WAS USING A KNIFE TO CUT A PIECE OF TUBETHE KNIFE SLIPPED AND CUT HIS LEG.
INJURED WAS WALKING THROUGH THE PLANT AT A HURRIED PACE. THE FLOOR WAS WET FROM EITHER WASHING DOWN OR FROM A RAIN STORM. AS HE STEPPED ON AN INSULATING MAT(BELT) HIS LEFT FOOT SLIPPED OUT FRO M UNDER HIM CAUSING HIM TO FALL. AS HE FELL, HE TRIED TO CATCH HIMSELF WITH IS LEFT ARM CAUSING HIM TO STRAIN LEFT ARM AND SHOULDER.
INJURED WAS USING A PIECE OF PIPE TO UNPLUG THE HOPPER TO THE DRYER FEED ROLLER. THE PIPE TRAVELED FARTHER THROUGH THE HOPPER THAN EXPECTED CAUSING HIM TO STRIKE A PIECE OF CHANNEL IRON WITH HIS HEAD.
INJURED WAS WALKING FROM THE LUNCHROOM BACK TO HIS DOZER. HE STATED HE WAS WALKING CLOSE TO THE HEAVY EQUIPMENT SHOP WALLS TO STAY OUT OF THE WIND. AS HE WAS WALKING HE STEPPED ON A SNOW COVER ED PIECE OF BELT CAUSING HIS FEET TO SLIP OUT FROM UNDER HIM CAUSING HIM TO HIT HIS RIGHT ELBOW.
INJURED WAS LIFTING AND ROCKING A BOAT OUT INTO THE WATER AND FELT PAIN IN HIS LOWER BACK.

INJURED EE HAD JUST COMPLETED RESTOCKING THE RIGHT SWING WERCH. UPON COMPLETION HE TURNED TO STEP DOWN FROM THE LADDER BEAM TO A SECTION AT FLOOR GRATING. THE EE'S FOOT MISSED THE GRATING CAUS ING HIM TO FALL AND STRIKE HIS RIBS & LOWER BACK AGAINST THE LADDER JACK PIN.
INJURED DRIVER RECEIVED ELECTRICAL SHOCK FROM THE CONTROL BOX, WHILE LOADING HIS COAL TRUCK.
INJURED WAS DISMOUNTING THE 988B LOADER WHEN HIS FOOT SLIPPED OFF THE LADDER CAUSING HIM TO FALL INTO THE LADDER BRUISING HIS LEFT RIB CAGE.
