Showing all 3Employee was walking down a slope behind our electrical building when ee stepped on wet leaves and mud, ee slipped and fell landing on left elbow shoulder area causing chronic irregularity of the clavicle from old injury ee had before EE came to work for us.
Employee was performing maintenance on #2 screen. Using a pry bar on counter weight when bar slipped. Felt the right shoulder pop. Employee is the superintendent for the welding team. At the time of the accident they had no lost time or restrictions. Later MRI was done and found a torn rotator cuff. Surgery on November 28, 2018.
