Continuous miner lost power, was lifting miner cable out of mud to check cable splices, while lifting cable out of mud, right foot sumped up and went to pull it out and twisted and felt something pop in left knee.

Roof fall occurred in the number 5 entry between spad 78 and 88, in a area that was mined in excess of 10 years ago. The area showed no evidence of taking weight or roof cracking prior to the fall. The fall occurred 18 crosscuts from the surface in the intake entry with the cfm excess of 250,000 exposing the roof to atmospheric weathering and temperature changes.

Employee was dragging feeder cable with ride, when EE stood up and twisted left knee.

Working on #6 Scoop. Employee was pulling cable leads through rubber grommet with channel locks when they slipped loose and channel locks struck above right eye. Employee was taken to ER were EE received 3 stitches.

IE and another EE were throwing miner cable and waterline against rib in crosscut between #3 and #4. When slinging the cable and waterline IE felt a pain in lower back. IE worked on Jan. 3rd and 4th and did not seek medical attention until Jan. 6th.

Installing belt clamps to make belt splice, hammering two wedges between clamp and belt. Top wedge broke causing hammer to deflect and struck injured in right cheek below corner of right eye approximately 1 inch, causing a 1 inch laceration. Taken to a medical center requiring 2 stitches to close laceration.
