The EE stated: while driving a 4 man diesel EE ran upon a small gob pile, while backing up off the pile, the steering wheel spun around striking EE's left wrist
The EE WAS OPERATING A MINER LOADING COAL IN THE #1 ENTRY. WHILE WAITING ON A CAR TO COME BACK AND LOAD A ROCK FELL FROM THE TOP, HITTING THEM IN THE UPPER MIDDLE PART OF THEIR BACK.
The EE was standing on a small gob pile dropping a miner cable, the gob pile shifted under EE's feet straining EE's right knee
THE EE WAS CUTTING A PIECE OF BELT. THE KNIFE SLIPPED, CUTTING EE'S LEFT ARM.
The EE was lifting a 5-gallon fuel can and felt pain in EE's lower back.
the employee was bolting in the #7 entry on the #3 unit. EE was tramming EE's bolter over a large rock that was laying on the mine floor. the rock shifted hitting the top of EE's left foot.
Employee was installing bottom belt rollers when EE experienced pain in EE's lower back. Employee began missing work on 1/10/24.
The employee was standing on the opposite side of the operator of the bolter on the left side of #3 unit. EE was at the back of the bolter next to the rear tire when the bolter trammed up aprox 6 " and ran over EE's toes on EE's right foot. there were no bones broken. the employee did not start missing any work time until 01/02/2024
The employee was cutting a piece of curtain with a belt knife, the knife slipped cutting their left thumb.
The Injured employee was driving a golf cart down the slope. as EE neared the bottom EE failed to maintain control. the injured employee exited the moving golf cart and it ran over EE's left foot.

The employee was removing a fire suppression canister for a scoop charger out of a scoop bucket, it caught EE's right pinky finger between the canister and the charger causing an abrasion
The employee was climbing out of the deck of a scoop. They put their left foot on the ground and felt pain in left groin.
Employee was installing roof bolts when a rock 2'X 3'X 5" fell out and struck EE in the right shoulder. The rock then rolled off EE's shoulder and landed on EE's foot. The employee immediately felt pain in the foot and shoulder.

The employee was bolting on the #3 unit in the #9 entry. EE was attempting to connect EE's top steel to EE's bottom steel when a piece of rock hit EE's upper left bicep causing EE to receive sutures
A roof fall was found at approximately 11:45 A.M. on the 215 Submain West Spad #19+50 #7 entry Primary Escapeway. The time of the fall is unknown. There were no injuries due to the rockfall.
A ROCK FALL OCCURRED ON THE 1ST NORTH BELTLINE AT XCUT 26 IN THE #6 ENTRY BETWEEN SPAD 16+80 AND 17+50. THE FALL WAS 70X18X12 FEET. THERE WERE NO INJURIES DUE TO THE FALL.
Employee was pulling packing from a pump, got their feet entangled in the stand causing them to fall backwards striking the pump shaft and a H beam.

A ROOF FALL WAS FOUND AT APPROXIMATELY 8:30 PM ON THE 1ST NORTH MAIN BELT BETWEEN SPADS 17+50 & 18+20. THE FALL WAS LOCATED IN THE #6 ENTRY. THE TIME OF THE FALL IS UNKNOWN. THE FALL WAS FOUND DURING THE EARLY TUESDAY NIGHT PRESHIFT. THERE WERE NO INJURIES DUE TO THE ROCKFALL.
A ROCKFALL OCCURRED IN THE #6 ENTRY (BELT) ON THE 2ND NORTH MAINS AT SPAD 54+20. THE ROCKFALL WAS APPROXIMATELY 18X40X10 AND THERE WERE NO INJURIES FROM THE ROCKFALL.
EMPLOYEE STATED EE WAS DROPPING THE PINNER CABLE ON THE #2 UNIT WHEN IT CAUGHT ON A PIE PAN, WAS STRETCHED TIGHT AND PULLED APART STRIKING EE IN THE LEFT HEAL. EE WAS WORKING ON THE POWER MOVERS CREW.
EMPLOYEE STATED THEY WERE IN THE BATTERY SHACK, STANDING IN A HIGH SPOT WHEN THEY TURNED TO LEAVE AND RAN INTO A LOW SPOT WITH THE SIDE OF THEIR HEAD CUTTING THEIR LEFT EAR. EMPLOYEE RECEIVED 8 SUTURES IN THE LEFT EAR.
EMPLOYEE STATED EE WAS CARRYING A BUCKET OF MINER BITS TO THE MINER WHEN EE FELT PAIN IN EE'S RIGHT SHOULDER WHEN EE SWUNG AROUND TO PUT THEM ON TOP OF THE MINER. EE IMMEDIATELY FELT PAIN IN EE'S RIGHT SHOULDER AND NECK. ACCIDENT BECAME REPORTABLE ON 10/31/22.
EMPLOYEE WAS BOLTING IN THE #3 ENTRY ON THE #2 UNIT WHEN EE TRIED TO PUSH THE ROOF BOLT UP ENOUGH TO GET IT IN THE CHUCK WHEN EE FELT PAIN IN EE'S LOWER BACK.
EMPLOYEE STATED EE WAS CARRYING A TOP ROLLER WHEN EE TRIPPED OVER SOME LOOSE CABLE BOLTS AND FELL INTO THE DIESEL MANTRIP. EE'S LEFT INDEX FINGER WAS CAUGHT BETWEEN THE ROLLER AND SIDE OF THE MANTRIP CAUSING A CUT AND ABRASION TO EE'S INDEX FINGER. EE HAD 3 SUTURES PUT IN EE'S LEFT INDEX FINGER.
A rockfall occurred on the 3rd west mains at spad #49+10 in the crosscut between the #5 and #6 entries. The fall measured 18x12x55. The fall was found around 6:00p.m. and was immediately called in on the MSHA hotline. No one was injured due to the rockfall
EMPLOYEE STATED WHILE THEY WERE LIFTING THE LIDS ON A RAM CAR BATTERY (GREEN BATTERY) IN THE #3 UNIT BATTERY BARN THEY SLIPPED ON A WET ROCK AND TWISTED THEIR LEFT KNEE. THIS ACCIDENT BECAME A REPORTABLE LOST TIME WHEN THEY WERE TAKEN OFF WORK 10-10-22.
