Showing all 13Third party delivery driver of raw materials was found deceased trapped between the frame and dump bed of EE's I-Bar owned and operated delivery truck. The driver was located on the West edge of the iron ore stockpile.
Distribution employees were moving railcars in the Canyon and noticed a grass fire, fire department was called after it started spreading. No injuries and no damage to any equipment. Fire put out in approximately 50-55 minutes
Employee was trouble shooting an FK pump and got dust in EE's eye causing EE's eye to get scratched. Prescription eye drops were prescribed (no work restrictions)
Employee was walking back to the main office in the main parking lot and stumbled causing them to fall, they landed on their right hand and fractured it "boxer's fracture".
Employee was working on K1 tower plug and some material got on EE's neck causing a minor burn.

Exiting loader and slipped on last step of ladder and fell on ground causing a fracture to upper end of humerus
EE was removing cutting edge from belly pan of a 631 scraper. While removing the last bolt the edge popped off the pan, fell & struck the employee in the hard hat & safety glasses. The impact caused a laceration to the left eyebrow & cheek from the top & bottom shields of the safety glasses that required 16 sutures.
Employee was cleaning and got some dust in right eye, washed eye out that day. Went to work next day and said eye was irritated, took employee to clinic and they washed the eye out. No prescriptions were prescribed.
The injured party parked in a tight parking space/caught tip of right index finger in the truck door smashing the finger nail / EE received two numbing shots to the tip of finger to enable finger nail to be cleaned, then put splint and wrapped.
