Showing all 4Employee stated that while attempting to drain oil out of the 34103-rock truck, EE stepped into deep mud and fouled EE's foot. When EE attempted to pull free from the mud, EE heard EE's knee pop. Employee was treated by an orthopedic surgeon and taken off work pending surgery. Medical data was made available to the company on 3/31/26 that Part 50 requires to report.
Employee was walking down a slight grade when EE slipped and fell, injuring EE's right shoulder. EE returned to work the next scheduled shift, under restrictions for 2 weeks. This is reportable due to EE's job change for the 2 weeks of restrictions.
While descending ladder on loader, employee misplaced their foot onto the outside bracket of a step; leading to rolling of the ankle and then falling to ground.
