THE EMPLOYEE WAS LOOSENING THE BOLTS ON AN ENGINE. HE WAS USING A RACHET TYPE WRENCH WITH A SOCKET WHEN A MALFUNCTION OCCURRED IN THE RACHET PORTION OF THE WRENCH, THUS THE EMPLOYEE SUSTAINED A STRAIN IN THE LOWER SECTION OF THE ABDOMEN ON THE RIGHT SIDE. THE CONDITION WAS DIAGNOSED AS A HERNIA AND SURGERY WAS NECESSARY.

LAID A SECTION 2F14 INCH PIPE ON FLATATION TANK TO PUT A FLEXABLE RUBBER CONNECTION THE END OF PIPE. THE PIPE ROLLED OFF THE FLAT TANK AND HIT LEFT LEG AND ANKLE.
