Employee was operating a Front End Loader for several hours. At the end of EE's shift EE felt sore. On 4/30/25 we were notified that the Employee sought medical treatment. EE was Dx with a Left shoulder strain and was given restrictions. Employee was moved to Light Duty. Reporting within 10 days.

Showing all 11Employee was descending from the Superior Guardian Tandem 7203 Screen Plant (Unit #273) using the ladder when EE felt a large pain in EE's left knee and it locked up. EE continued working until around 10 a.m. when EE decided EE needed to have it checked out. Per the doctor EE sprained EE's left knee from planting wrong while descending from the Screen Plant.
On January 19th, 2022 at 8:29 am rocks came off the east highwall striking the employee from approximately 38-42 feet up the highwall. The employee sustained multiple injuries to their head, arm, and leg.
Employee entered dozer cab to verify need for fueling, while looking at gauge door blew shut by a gust of wind trapping left fourth finger causing broken nail and crushing injury to left fourth finger.
Employee climbed step ladder to fuel Roll Plant in quarry. When descending, both feet on ground, one foot was on top of a rock. EE pivoted & foot on rock caused to lose balance, did not fall. EE was able to walk on the foot. Told supv. that they "took a digger". After seeking treatment, determined to be a R ankle sprain. We accommodated light duty and there was no lost time.
The injury occurred when a trained miner fell from a height of 4 to 5 feet onto the rock floor of the quarry. The condition contributing to the fall was the miner placing himself in an elevated position with no tie off. There was one injured party.
Fell on ice patch while walking around a conveyor, one foot on gravel other on ice patch, struck right shoulder on ground.
A ground person was assisting the operator of a skid steer in a task that required the ground person to hold power cords up and away from travel way of skid steer. Skid steer oper. toured sharply to the right causing rear left co new to come in contact with ground person right forearm causing it below the elbow
WHILE ASSISTING TO FREE AN OVERSIZED ROCK IN THE PLATE FEEDBOX AT PRIMARY CRUSHER, HIS FINGERTIP WAS PINCHED BETWEEN CHISEL AND SIDE OF FEEDER WHEN THE STONE CAME LOOSE.
WHILE REMOVING A WORN PIECE OF SCREEN CLOTH FROM SCREEN BOX, THE OPERATOR TURNED TO RELEASE SCREEN PANEL TO ALLOW IT TO FALL TO THE GROUND. WIRE BECAME ENTANGLED IN OPERATORS GLOVE DRAGGING HI M TO THE GROUND.
OPERATOR WAS IN THE PROCESS OF SHUTTING DOWN PIECE OF EQUIPMENT WHEN HE SLIPPED ON A SNOW COVERED ICED OVER SUPPORT IRON FOR CLUTCH ASSEMBLY STRIKING HIS LEFT SHOULDER.
