Employee reached under a load suspended by an all terain fork lift when the load shifted and fell pinching the employee's left hand. The resulting injury to the palm side base of the employee's left thumb required three sutures to close.
Contract trucker working for customer, K.C. Trucking, used a tire gauge to realign the fan belt on his truck with the engine running. In the process three finger/tips on his left hand were cut off. Injured trucker was taken to hospital and is now home recuperating. K.C. Trucking does not have an MSHA contractor ID#. K.C.Trucking: phone # 763 420-6116.
Employee got his finger between slide bar and spinner pinching his index finger of his left hand. EE thought that he had only damaged his fingernail. A band aid was placed on it and the EE continued to work. EE went to the Doctor on Oct. 26 and was diagnosed with a fracture at the tip of his index finger. No treatment was needed.
Employee alledges he was shoveling stemming material when his back "locked up."
While unlocking a camlock fitting on a pit pump the employee was pulling on the 4 inch female fitting trying to get it lose. When the fitting came loose it struck the employee hitting between the nose and upper lip. *** Resulting in four sutures to his upper lip - justifying a reportable medical injury with no restrictive duty or lost days***
Employee was taping a hole in a steel plate when the tap broke. The employee suffered a laceration to his right hand when he contacted the broken stub of the tap still in the hole in the plate. This wound required multiple stitches to close. Employee was not wearing gloves at the time of his injury.
Technician was using a cutting torch to cut a relief hole in a tie rod on a 769 Haul Truck slag from cutting operation blew back into employee's right eye.
EE was installing a new chain sprocket on the digger drive. After the completion of the project, EE felt pain in his right hip. Plant manager was not notified of injury until 3/15/2006. EE was seen by Dr. & recommended to have MRI and be seen by surgeon. EE underwent surgery on 3/21/2005 for herniated disc.
DRILLER WAS PUTTING A NEW SAVON SUB ON THE ROTERY HEAD. WHILE TRYING TO TIGHTEN THE SUB INTO THE HEAD, DRILLER WAS HOLDING BREAKOUT WRENCH (48" WRENCH) ON THE SUB AND RUNNING CONTROLS WITH HIS OTHER HAND. WHEN HE HIT THE ROTATION LEVER THE WRENCH KICKED BACK AND SMASHED HIS LEFT INDEX FINGER BETWEEN THE WRENCH AND THE MAST.
Employee was operating a 966c loader. He went into the muck pile to get a bucket of material and the front wheels of the loader fell into a soft spot and came to a abrupt stop. The employee sustained a whiplash. He was wearing a seat belt. His conditioned worsened and he was given restrictions on 11/4/05.
Employee was repairing a hose on the water truck. He turned around to get a wrench from the service truck, took a couple of steps, and apparently stepped on a rock and twisted his left knee, causing a sprain.
EE & MANAGER WERE TRYING TO REMOVE A 4" CAP ON WATER LINE USING A 36" PIPE WRENCH & A 8LB MAUL TO TAP ON THE WRENCH. THE WRENCH SLIPPED OFF OF THE CAP & PINCHED THE EE'S FINGER REQUIRING 4 STITCHES.
When retrieving video camera after a blast, slipped and fell on uneven, loose ground material.
Employee attempted to assist in lifting a wear plate and popped something in his back/hip area.
EE claims that the brakes failed at the top of the slope leading into the pit. EE was on his way to get his 9th load of the night. EE had conducted pre-shift on the haul unit. The vehicle str4uck the highwall as he attempted to negotiate a curve and ramp leading out of the pit.EE received a laceration to left forehead area and was hospitalized.
EE was getting off a 980 f loader and when he got to the last step, he put his foot on the ground. he didn't see a pothole and his right ankle twisted and his weight went down on it. He went to the hospital and they said it was a bad sprain. Became a lost time on 6/17/05.
EE WAS USING A HOIST CHAIN TO MOVE A METAL DOOR TO REDIRECT SCREENINGS. HOIST CHAIN HOOK SLIPPED OFF OF THE METAL DOOR AND HOIST ARM STRUCK THE EE IN THE MOUTH, CUTTING HIS LOWER LIP. REQUIRED STITCHES.
Employee was being lowered in manbasket. Basket stopped suddenly. Employee was standing with legs crossed lighting a cigarette when basket stopped causing him to fall in squatted position. Knee struck basket interior causing injury.
Employee was making a plant change by opening a gate when he smashed his finger between the handle and a keeper pin. The employee received 4 sutures.
Employee was using a utility knife to cut the end of a fire hose and the knife slipped cutting his right index finger.
Ee was installing a saddle onto the mainframe arm of the omnicone 1560 crusher and caught the end of his middle finger on his left hand between the saddle and the frame arm removing the skin degloving from the tip of the finger. Dr treatment included skin graft making this a reportable medical injury with no lost workdays.
Employee was cutting plastic ties and cut his arm with the knife. He was cutting in the direction of his body. Doctor used eight stitches making this a reportable medical.
EMPLOYEE WAS WORKING OUTSIDE ON UNEVEN GROUND. HE TWISTED HIS RIGHT ANKLE. HE REPORTED THIS TO HIS SUPERVISOR BUT DIDN'T NEED MEDICAL ATTENTION. HE WENT TO THE DOCTOR ONE MONTH LATER AND FOUND THAT HIS RIGHT FOOT WAS BROKEN. THIS BECAME A LOST TIME MEDICAL ON 6/10/05.
Employee was removing the teeth on a backhoe bucket using a hammer. A metal fragment broke off and entered the employee's left shoulder requiring a physician to remove it.
Employee lifted hood on EX270 to check oil. Hood did not catch and lock and it fell hitting him on head. Employee was sent to Dr and released as 1st aid. He complained on 4-12-05 and requested to be re-sent to Dr. He was put on light duty pending x-ray results.
OPERATOR WAS TRYING TO GET UP ON A DOZER, HIS RIGHT FOOT SLIPPED. HE TRIED TO CATCH HIS FALL CAUSING HIS RIGHT LEG TO HIT THE PUSH ARM, HIT SHIN BONE AND SKINNED IT WHICH LATER GOT INFECTION,CELLULITIS REQUIRING MEDICAL ATTENTION. INVESTIGATOR SAID DOZER PUSHARM NEED TO BE FIXED AND HAD BEEN REPORTED. IT WAS LEANING AND OP HAD TO STEP ON IT FIRST
Employee was assisting in the removal of the spider cover plate from the gyrator crusher. When the plate was lifted, it began to swing and the employee attempted to stop it from hitting the bulk oil tank. His hand was pinched against the oil tank.
A large roof fall occured in DD @ 11 intersection, luckily the scaler or any other personnel were not in the area at the time. The operator had intentions of scaling a large rock when the machine broke down on the 27th.
THE EMPLOYEE WAS STEPPING OFF OF A WORK PLATFORM & MISSED THE STEP. HE STARTED TO FALL & REACHED OUT TO CATCH HIMSELF WITH HIS RT. HAND. HE BROKE HIS RT. WRIST WHICH HAD PREVIOUSLY BEEN BROKEN IN A WORK RELATED ACCIDENT & WAS STILL IN A CAST.
Employee was holding a spud bar while another employee was striking the bar with a sledge hammer to break a rock from crusher. The hammer missed the spud bar and struck the employee's left wrist.
Employee was checking a flex conduit at 2 jaw. He had his hand on a beam and a rock fell from above. The rock hit his left index finger causings a cut.

Employee was guiding cylinder into place with a come along in 773B Cat truck. While jacking into place, cable around cylinder slipped & caught fingers between cable & ride cylinder. Cable had total weight of cylinder. Avulsion of 2nd finger on right hand to first knuckle & fracture of 3rd finger.
Employee was attempting to clean screen deck. In the process, his hand struck a bolt on the deck and the sharp edge cut his finger.

AFTER COMPLETING THE PRE-SHIFT INSPECTION OF HAUL TRUCK, EE REMOVED THE WHEEL CHOCK AND WAS THROWING IT UP ONTO THE CAB WALKWAY OF THE TRUCK WHEN EE FELT A PAIN IN LOWER BACK. EE WAS DIAGNOSED WITH A MUSCLE STRAIN AND RETURNED TO WORK

EE WAS STANDING ON THE GROUND REMOVING CHAINS AND BINDERS FROM A LOW-BOY TRAILER IN PREPARATION TO LOAD A TRUCK. ONE OF THE CHAINS HUNG UP ON THE TRAILER AS THE EE PULLED ON IT. EE FELT A "POP" IN RIGHT SHOULDER
Employee was climbing access ladder on a WA800 front end loader when he fell to the ground. Employee right foot slipped on second rung from cab platform and he lost his grip with right hand. He swung out to the left while left foot and left hand still on loader when he lost his grip and fell an estimated 8.5 feet to the ground landing on his right side.
Employee was rigging a head pulley for a lift when he was contacted by the boom of the lifting device!
Employee was turning the water on at the water stand pipe so he could wash the bed of the truck bed. When he bent over to turn the water valve on there was an aluminum valve wrench hanging over head approximately six foot off the ground. The wrench fell striking the employee's hardhat and back of his head.
EE cut right hand open when he slipped and fell walking to begin shift. the ee was crossing a wooden bridge from ee building to the stockyard and slipped on the wet/ icy bridge. When he braced against the fall, his hand struck the planks of the bridge and cut open. The ee was transported to MedFirst at 8 AM for treatment. required stitches.
EMPLOYEE WAS ATTEMPTING TO DRIVE VEHICLE AWAY FROM EMPLOYEE ROOM & ANOTHER VEHICLE WAS PULLING AWAY AT THE SAME TIME. 2ND VEHILCE SPUN WHEELS ON ICE AND DIRT FLEW IN THRU EE'S OPEN WINDOW & STRUCK HIM IN THE RT. EYE, CAUSING AN ABRASION.
WHIILE DISEMBARKING FROM RT SIDE OF LOADER, WELD ON PLATFORM BROKE. EE THOUGHT THE PLATFORM WAS COMING OFF AND JUMPED BACKWARDS LANDING WITH FRLL WEIGHT ON LEFT LEG.
