EE was climbing an 8ft. ladder when the ladder slid downward and his left leg fell through the ladder and landed on the ladder with his left leg underneath the ladder step. This accident was not reported to the shift supervisor at the time it happened, it was not until the 8th of May in the morning when he stepped on a rock that he asked to leave work to go to the clinic.

THE 50" RAYMOND MILL WAS BEING TAKEN APART TO REPLACE THE MILL BASE. THE 5 ROLLER JOURNALS HAD BEEN REMOVED AND SET ON THE FLOOR. EE WALKED BY AND BUMPED INTO ONE JOURNAL CAUSING IT TO FALL OV ER BEFORE HE COULD MOVE OUT OF THE WAY. THE JOURNAL FELL ON HIS LEFT FOOT SMASHING HIS STELL TOE DOWN FRACTURING ONE OF HIS TOES.
EE WAS OPERATING FORKLIFT WITH FOOT HANGING ON ONE SIDE OF THE LIFT. FOOT STRUCK A PALLET OF BAGS OF CLAY TWISTING HIS FOOT & CAUSING SPRAIN
A 55 GALLON BARRELL FULL OF WATER FELL AND AS HECAUGHT IT TO KEEP IT FROM TURNING OVER, HE FELT HIS STOMACH RIP WITH A BURNING SENSATION.
EMPLOYEE WAS HELPING INSTALL A GEAR BOX ON ELEVATOR SHAFT. BOX FELL AND CAUGHT EMPLOYEE'S RIGHTRING FINGER BETWEEN GEAR REDUCER AND GUARD SUPPORT AND TORE OFF FINGERNAIL.
EMPLOYEE PUT UP A LADDER TO CLIMB ONTO A HOPPER BIN. LADDER SLIPPED OUT FROM UNDER HIM CAUSING HIM TO FALL AND HIT HIS SIDE ON HOPPER BIN.
