EE was attempting to lift handrail which had been tack welded to the floor. When they lifted the item with the crane, the steel floor lifted with it, they stepped forward to check why the handrail was not lifting and their right foot ended up underneath the floor plate as the weld failed and the plate fell onto their foot causing a fracture.

EE was taking a track pad out of equipment, and believes they injured themselves when they were knocking the pins out of the track.
EE was walking inside the revolving frame of the dragline and stepped on one of the floor lids, causing EE to lose EE's balance. EE fell and hit EE's side.
EE stepped out of the lull (left leg first), and did not realize they were stepping off into a low spot or hole. As the EE exited, they rolled their left ankle.
EE was assigned to drain and refill the jaw crusher oiler, as EE was holding the bucket in place, EE felt EE's lower back give out.
Employee was doing daily inspection on jaw crusher, when a haul truck dumped its load at the top of the ramp. The material surged from the primary into the grizzly feeder and then a rock came out and struck employee on the back of the hardhat.
EE was working on loader seat and got hand pinned between scissor frame.
EE was stepping off the work boat onto the dredge when EE's foot slipped through the handrail of the work boat, pinning EE's leg up to the knee. EE didn't go to the clinic until 2/18/2025, they said EE has an infection in the lower part of the leg
Employee hit a troughing roller on a conveyor with a hammer and struck hand fracturing EE's right thumb and index finger, as well as lacerating EE's right index finger requiring seven stitches.
OVERHEAD ROCK, ROCK FELL AND STRUCK RIGHT HAND.
A large rock lodged in conveyor at head pulley, employee got on belt and kick belt rock out, when EE did the rock went out and EE's leg was trapped between the belt and head pulley, EMS showed up and cut the belt to get EE's leg out, the counterweight dropped and through them off and they were able to get EE's leg out.
EE was working outside and stated didn't feel well.
Employee was using a drill to tighten the nuts on the lower side of the apron of a crusher when the load moved sideways to the ground catching the employee's finger causing a laceration and fracture.
Employee was driving the forklift, hit a bump, bounced in the seat, and struck left testicle between employee's leg and the seat.
Employee was holding a pipe with left hand and a 4 1/2 angle grinder with right hand when the grinder kicked back striking the employee's left thumb.
The technician was repositioning a large trunnion cap for cleaning when it slipped from their grasp pinching left thumb between the cap and the catwalk. The Supervisor called Work Partners, they advised them to go to the hospital. The Foreman from the day-shift took them to the hospital where they were found to have a fractured thumb tip and received five stitches.
While working on conveyor employee hurt their left arm.
Employee removed a worn crusher bar from the crusher and when setting it down on the frame of the motor base employee felt the right side of back give away.
Employee jumped from a 992G loader in a panic thinking it was on fire injuring both heels. The loader was not on fire after turning the equipment off.
Employee was dismounting a small excavator at night and lost balance on the track and fell 2.5' to the ground breaking 4 ribs.
EE was operating a John Deere 310E articulating haul truck when they hit a pothole causing the seat to bottom out and cause back pain.
Employee was attempting to remove a wheel chock when the employee lost footing slipping in the mud falling at the same level.
Employee's pinky was pinched between a winch block and the dredge ladder.
An employee was operating a CAT 390F excavator in the South Quarry Pit. The machine went into the water and sank to a depth of approximately 25 feet. Consistent with 103K order modifications approved by MSHA, the operator was recovered the following day.
Employee's pinky finger was caught between a bucket tooth and the fork of a telehandler.
Employee was performing a maintenance task that resulted in an eye irritation injury.
Heart attack at facility, natural causes.
Loader operator noticed an A/C vent fell out of housing. While replacing A/C vent while A/C was running, finger closest to thumb on (L) hand encountered fan cutting finger tip.
Employee was helping another employee set a part on the rear end of a maintenance truck. The employee set the part down on right ring finger. The employee was taken to the doctor and diagnosed with a closed fracture of tuft of distal phalanx of finger.
Employee was climbing a step ladder when it broke. The employee fell approximately 3 feet to the ground spraining ankle.
While lowering a large piece of steel during dragline repair activities the steel hung up, and the injured miner moved in to help free the load. When the load became free it shifted towards the miner, striking them on the right shoulder and upper chest area
Employee reported back was hurting after washing off the dragline tracks. This injury did not become MSHA reportable until employee received treatment on 1/12/2022.
Employee was removing a pin connecting the portable crusher to the loader. When tapping out the pin with a hammer and pry bar, the tongue of the portable crusher moved towards the employee causing the pry bar to lacerate the employee's left index finger. The employee required stitches to treat the laceration.
Customer Truck Driver, Not a Miner or Contractor: A customer truck driver jumped from the top of the trailer to the ground in an attempt to confront an employee of the mine that was telling customer to get down off the top of the trailer. The driver admittedly knew it was not allowed to climb on the trailer but did so anyway.
Employee was changing blow bars on a HSI crusher. EE used a small sledge hammer to hit one of the blow bars causing a piece of the blow bar to chip. The piece hit EE just inside EE's right nostril. The employee was taken to the emergency department where the piece of metal was removed and EE was given one stitch.
On 8/11/2020 the employee hit right knee on a metal lip while climbing into screen deck to check screens. The employee reported the injury on 8/25/2020 when employee requested to see a doctor and was diagnosed with a knee sprain.
A Customer Truck Driver had an allergic reaction to a Bee sting.
Employee was working on the drill and when EE was finished EE stepped off the counter weight onto the grate platform and slipped and twisted (L)ankle. Employee went home to rest it figured it would be ok and while at home it started to get worse that's when EE decided to see a doctor on 1/18/20.
Employee's left knee became sore after operating 2 pieces of equipment
The employee was pulling the rubber door back on the screen deck when employee felt a pain in right arm resulting in a strain.
On approximately 7-15-2019, an employee felt a burning sensation in abdomen after moving an object. The employee did not wish to seek medical treatment. On 3-3-2021, after informing manager that the condition had worsened, the employee was sent to a doctor who placed the employee on work restrictions.
The employee was struck on the shin by a tail pulley that slipped out of another employees hand.
While dumping material out of the truck, a customer truck driver caught EE's finger in the tailgate resulting in a laceration.
The employee struck knee on a piece of metal while accessing the dragline resulting in a contusion. EE received medical treatment on 4/30/2019.
IE was moving the portable crusher by remote control when a small piece of metal broke from an undercarriage roller and struck abdomen. IE transported to hospital where a 2nd piece of metal was found in abdomen from 20 yrs ago. Dr decided newer piece of metal could remain but recommended removal of older piece for fear of working into bowels. Dr removed metal, not sure which piece.
IW strained right elbow while pulling water hose up stairs. IW was given restrictions on 11/05/2018 which IW was unable to do job duties at full capacity.
The employee was injured in the left hand and leg when EE was struck by metal chips from a hammer.
Death due to Natural causes.
Employee suffered a cardiac arrest while dismounting front end loader.
