Showing all 4Setting up the plant, operator of end loader had placed the conveyor hopper on blocks. They backed up and the chain hook caught the conveyor/hopper and dropped on injured employee's foot. EE had steeled toe boots on and got no broken bones, but the toes were pretty squashed. EE had stitches and developed compartment syndrome in their calf.
A boulder was stuck in the jaw. Employee shut down the power source, locked out, put on his safety harness, climbed up on platform, tied himself off removed handrail. He then attempted to put on the chain to extract the boulder. Realized the jaw was not in the resting position, jaw rolled over and pinched his finger.
Troughing rollers had stopped. Employee pounded with a hammer and hammer got caught.
