Employee claims they slipped and lost footing and fell and hit their head on the ground.
Setting up the plant, operator of end loader had placed the conveyor hopper on blocks. They backed up and the chain hook caught the conveyor/hopper and dropped on injured employee's foot. EE had steeled toe boots on and got no broken bones, but the toes were pretty squashed. EE had stitches and developed compartment syndrome in their calf.
Troughing rollers had stopped. Employee pounded with a hammer and hammer got caught.
