EE was helping to lower a 75 hp. fan from an elevated platform, approx. 7ft. The fan slid off of the cables and fell to the platform. When the fan fell, it hit him and knocked him off the platform onto the conveyor belt, hurting his back and left thigh. Fall protection was available but not used.

As employee was descending out of the cab of an excavator onto the boarding steps, employee lost his footing and slipped off of the bottom rung. Employee held onto the boarding handle with his right hand to prevent his fall but injured his shoulder in the process. Pain persisted in shoulder, ultimately required surgical remedy.

The employee stated that a small particle entered his right eye when he took off his glasses to clean them. He believes the particle came off of his glove. The particle was removed by a doctor and he was given an antibiotic to prevent infection. Upon return, the employee continued his normal work.

Employee was performing ring drilling. When inserting the drill steel and bit, steel fell from the centralizers. Employee attempted to control the falling steel. The falling steel pinched the finger between the steel and frame resulting in laceration of the right index finger requiring sutures. Treating physician did not prescribe any time away from work for injury.

The employee was tightening a 1in. ball valve with an 8 in. pipe wrench. The wrench slipped with extreme force. When this happened, he felt a sharp pain in the shoulder area. The shoulder was still sore through November. The employee had surgury on Dec. 3 for a torn ligament.

Employee was installing a self tapping screw with a hand held drill. The drill was equipped with a threaded hole for a side handle which was not installed. When the screw "grabbed", the torque twisted his arm and shoulder. He continued normal maintenance activity until the day he went for shoulder surgery.

EE had finished unloading pipe and was rolling the sections out of the roadway, approx. 6ft. While rolling a 15ft. section of pipe, he felt a sharp burning in his lower right pelvis. He was evaluated at Taos Holy Cross where it was determined he had a hernia. EE resumed his normal duties until Friday, May 18, when he had surgury on the hernia.

At 1300 incline station, employee was changing rollers from LHD bucket. LHD operator prepared to dismount and install chocks. LHD rolled several feet striking a Kubota which struck the employee. Taken to doctor initially, doctor found only bruising. After employees normal 5 days off, went to another doctor who gave restricted duty. Restricted duty started 4/20/07

Employee was performing housekeeping when he attempted to move a portable metal gate by hand. He felt a pinching sensation in his groin area which was later diagnosed as a hernia. The employee performed work with a 20lb. lifting restriction until 6/5/07, the date of the procedure to repair the hernia.

Erica was working the grizzly lines. She was on the west side of window #10, line 9 attempting to move a rock with a shovel. She was using a "prying" motion when a falling rock caused the shovel to twist, straining her right wrist.

Substandard Equipment-Cabinet was supplied with a sharp edge. Employee reached into a box six inches from the sharp edge and contacted the edge with his finger. An avulsion resulted requiring 5 stitches to close.

Employee was bolting a heading with split sets. The split set driver momentarily stuck inside the set allowing the air to bleed off causing the Jack Leg to drop quickly. He tried to catch the falling Jack Leg. He felt a "popping" sensation in his left shoulder. Employee had surgery on his shoulder on 1/19/07.

Contractor employee knocked a lens cover off of a sodium vapor light when removing tools from a ball mill after completing his work. Lens struck his chin causing a small laceration which received sutures to close.

The electrical power to northern Taos County was lost during a storm. Our main hoist suffered damage to a rectifier that became evident about an hour later when 51 miners were coming to the surface at the end of their shift. Electricians troubleshot the hoist to find the problem and replaced the part. Hoist returned to normal ops at 5:40 pm and all miners came to the surface.

After removing his gloves for a better grip, employee was trimming the liner, cutting away from himself with his right hand while pulling the newly cut liner the opposite way with his left hand. This action resulted in a laceration at the base of the employee's index finger on his left hand. Laceration required 7 stitches.

The # 1 Hoist was down due to a power failure at the Tri-State # 69 substation caused by a bad jumper. The power failure effected all of the Molycorp facility. All of the underground personnel were evacuated from the underground mine. Power was re-established at 9:11 AM. The hoist systems were checked and found OK.

Hoist power problem due to power company signal system to the hoist sub station. The Westinghouse 4160 switchgear dropped out when it recieved the signal, the unit worked properly. The hoist was out of service for more than 30 minutes.

EE was working on a welder to adjust idle speed. As he worked the throttle linkage, his glove got caught in the fan belt and pulley. Ee left index finger was struck by fan place and cut severly. Dr amputated end of finger.
