Employee was moving crusher break plates in storage building. One of the plates started to fall over and EE pushed it back with EE's foot and felt a pop in EE's knee. EE went for treatment on 03/19/2026 and was diagnosed with a right knee injury and primary osteoarthritis.

Employee was cleaning the tracks of the bulldozer with a shovel. EE heard a pop and experienced pain in EE's right arm. Employee continued to work unrestricted until EE went to an orthopedic specialist on 12/30/2025. EE was diagnosed with a tear of the right rotator cuff. The extent of the tear is unspecified. The employee is now on restricted duty.

IP was attempting to find a leak in a hydraulic system on EE's repump truck. While moving a line, hydraulic fluid was injected into right hand index finger.

MCC building for primary crusher caught fire and burned down. The MCC was located in an area that did not damage anything else.

Employee did not seek treatment until 02/25/2025. A pin for the excavator bucket was being knocked out and when it came out, instead of letting it fall, the employee held onto it, not realizing the weight. Employee's left ring finger was smashed in between the pin and the excavator bucket. The employee received a fractured finger and two sutures.

Employees found the fall as they were entering into the mine at main 8 face out by. The fall area was approximately 40' long, 40' wide. MSHA was notified within 15 minutes of the fall being discovered. The 103 (k) order was modified by MSHA.

A roof fall occurred in 1R2 between crosscuts 7 and 8. The area was approximately 40'x 40'. MSHA was notified within 15 minutes of the fall occurring and the prior 103 (K) order was expanded.

Foreman was conducting workplace examination of the mine and discovered a roof fall at the intersection of 1R4 and 1RC10.The fall area is approximately 40' long, 35' wide, and 12' high. MSHA was notified within 15 minutes of the fall being discovered. A 103 (k) order was issued by MSHA.

One employee was handing a screen to another employee and the screen slipped from employee's hands. The injured employee was struck with the screen in the right pectoral suffering a chest wall sprain.

Employee was exiting CAT 777 rock truck to fill the oil. While climbing down the ladder, the employee's foot slipped off the ladder and employee suffered a rupture of the right proximal bicep tendon.

Employee was descending the steps from the Raymond Mill. They fell down the last two steps and landed on their knees. Employee suffered a partial ligament tear in their right knee. The injury did not result in any lost time or restricted duty until they went for surgery on 08/23/2023.

Employee was standing inby the roof bolter in Entry 1R6 looking for loose material to hand scale. A piece of stone, approximately 2'x2'x3" fell and hit the employee in the hard hat and right shoulder. EE received a shoulder contusion.

Employee was walking to place EE's timecard in the mailbox. EE was walking through snow from the prior night and EE slipped on ice that was covered by the snow. EE suffered a tear of EE's right rotator cuff.

It is possible that this employee contracted COVID after riding in a pickup truck with another employee who tested positive for COVID.

Employee sprained left ankle when EE stepped over a small stream of water and placed EE's foot down on un-level ground.

While changing steels on the roof bolter, the steel slipped from the employee's hand. It struck EE on the top of EE's left foot behind the steel toed portion of EE's boot. EE was diagnosed with a fracture of the 4th metatarsal bone.

Employee was installing a 7' roof bolt when EE felt a pain in the arm. EE was diagnosed with a left rotator cuff tear.

Employee opened the door to enter the water truck and a piece of metal fell into their eye. Employee had to have the imbedded piece of metal removed.

Employee strained back while cutting and resetting the hoist cable on the dragline. The employee did not notify foreman about the injury until 04/02/2021 after they went to see a physician. The employee was scheduled off on 04/02/2021. Employee finished shift on 03/31/2021 and worked on 04/01/2021 without notifying anyone of the injury.

There was a rock wedged between the dual wheels on the lube truck. After a few attempts at trying to remove the rock, the employee leaned over the tire and the inside tire blew out. The employee suffered a broken nose and facial bruises.

Employee was cutting a hose fitting off an airline and the knife slipped and lacerated EE's left pinky finger. EE received 3 sutures to close the laceration.

Employee was helping to remove a pin from a CAT 990 loader. When the pin released from the bell crank, it hit the frame, upended, and slid out of the supporting sling. It landed on the employees toes of left foot.

Employee was moving the belt guard for the scalping screen. When they turned with the guard, they felt a pop in the lower back. Employee suffered a back strain.

Employee was dismounting bulldozer. EE's foot slipped which caused excessive weight on EE's right shoulder. The physician believes EE suffered a torn right rotator cuff.

Employee was standing on the 3rd rung of an 8' step ladder, operating a chain come along, lowering a chute from a storage bin. When the chute was approximately half way between the bin and the floor, it shifted and knocked the employee off the ladder. The employee suffered a fractured pelvis.

Foreman was conducting workplace examination of the mine. Foreman saw the roof fall when foreman entered into Main 10 from Main 9. The roof fall is in by of crosscut 10. The exact date of the roof fall is unknown because the mine was unoccupied on 12/14/2019 and 12/15/2019. MSHA was notified within 15 minutes of the fall being discovered.

Employee was pulling a rope attached to a water suction hose and they felt their wrist pop. They were diagnosed with a wrist sprain.

Employee was lifting a 5 gallon bucket of oil and EE felt a pop in EE's left arm. EE was diagnosed with a torn bicep muscle.

Employee was exiting a CAT D6 bulldozer. When EE stepped onto the ground with right leg, EE felt a pop in the right knee. EE was diagnosed with a knee sprain.

Employee was pulling the fuel hose from the fuel truck to EE's drill. EE slipped and fell onto EE's back and suffered a compression fracture.

Employee was climbing down the ladder, exiting the haul truck when EE felt a pain in EE's chest. EE was diagnosed with a strained chest muscle.

Employee climbed into the foreman's pickup truck and while riding around, ee noticed swelling and pain in left knee. EE went to the ER that evening.

According to employee's physician, this is not a work related injury or illness. The employee was operating a stone drill when they began to have shortness of breath. An ambulance was called and employee was taken to the hospital and admitted. Employee was diagnosed with a bleeding ulcer and fluid behind the lungs.

Employee was not feeling well all day while operating the mill. EE began sweating and experiencing pain in arm. EE went to the Emergency Room and was diagnosed with having a heart attack.

Employee was unloading material from the back of the plant truck when employee felt pain in groin. Employee was diagnosed with a hernia. This was not a restricted duty or lost time injury until employee had surgery on 06/15/18.

Employee was prying on the lip shroud of a drag bucket. When the bar slipped, they strained their right shoulder.

Employee was operating haul truck when employee began to feel ill. Employee was transported to the hospital by ambulance. The physician stated that it was not a work related issue. This illness was due to high blood pressure which was a preexisting condition. The employee was taking medication for this condition, but it became ineffective.

Employee was bent over, looking under a catwalk in the Page dragline and when ee stood up ee hit head. EE received a cut on the top of head that required 6 staples.

Employee was using a breaker bar to remove a bolt on the wheel assembly on a Komatsu WA 800 when EE felt a pop in lower groin. EE was diagnosed with a hernia. EE remained on regular duty and this did not become an MSHA reportable injury until EE had surgery on 02/16/2018.

Employee parked truck on the scales and when ee went to exit the truck, ee slipped on the ice and hurt shoulder and back. The employee did not seek medical attention until 01/23/2018. This injury became reportable because ee was given a physical therapy prescription.

Employee was exiting excavator and their foot slipped off the step. This caused employee to lose balance and fall to the ground. Employee went to a physician on 04/24/2017 and was diagnosed with a broken rib.

Injured was installing tail pulley guard on the dump belt under the crusher. As they were trying to position the guard, employee suffered low back strain.

The employee was installing a transmission guard on a piece of Caterpillar equipment and caught employee's right foot between the guard and frame. Employee broke bone in top of foot. Employee did not seek medical attention until 3-5-2017.

A chain the employee was using to pull tracks for a dozer broke and struck the employee in the left forearm causing a distal radius fracture.

Employee was cutting a conveyor belt with a utility knife and the knife slipped and cut left arm. EE went to a physician and they glued the laceration closed.

Employee was lifting a bracket for a conveyor and EE felt pain in EE's abdominal area. EE went to see a physician on 04/06/2016 and a small sliver of metal was removed. EE was also diagnosed with a small abdominal hernia.

Employee was exiting a pickup truck when EE slipped and fell on ice. EE suffered a fractured right radius and a fractured rib.

Employee was changing gates on the dry screen when he suffered an umbilical hernia.
