Accident type, without injuries

MCC building for primary crusher caught fire and burned down. The MCC was located in an area that did not damage anything else.

Struck by falling object

One employee was handing a screen to another employee and the screen slipped from employee's hands. The injured employee was struck with the screen in the right pectoral suffering a chest wall sprain.

Fall from machine

Employee was descending the steps from the Raymond Mill. They fell down the last two steps and landed on their knees. Employee suffered a partial ligament tear in their right knee. The injury did not result in any lost time or restricted duty until they went for surgery on 08/23/2023.

Employee sprained left ankle when EE stepped over a small stream of water and placed EE's foot down on un-level ground.

Struck by falling object

Employee opened the door to enter the water truck and a piece of metal fell into their eye. Employee had to have the imbedded piece of metal removed.

Over-exertion in lifting objects

Employee strained back while cutting and resetting the hoist cable on the dragline. The employee did not notify foreman about the injury until 04/02/2021 after they went to see a physician. The employee was scheduled off on 04/02/2021. Employee finished shift on 03/31/2021 and worked on 04/01/2021 without notifying anyone of the injury.

Fall from ladders

Employee was standing on the 3rd rung of an 8' step ladder, operating a chain come along, lowering a chute from a storage bin. When the chute was approximately half way between the bin and the floor, it shifted and knocked the employee off the ladder. The employee suffered a fractured pelvis.

Employee was exiting a CAT D6 bulldozer. When EE stepped onto the ground with right leg, EE felt a pop in the right knee. EE was diagnosed with a knee sprain.

Employee was not feeling well all day while operating the mill. EE began sweating and experiencing pain in arm. EE went to the Emergency Room and was diagnosed with having a heart attack.

Employee was prying on the lip shroud of a drag bucket. When the bar slipped, they strained their right shoulder.

Struck against stationary object

Employee was bent over, looking under a catwalk in the Page dragline and when ee stood up ee hit head. EE received a cut on the top of head that required 6 staples.

Employee was exiting a pickup truck when EE slipped and fell on ice. EE suffered a fractured right radius and a fractured rib.

While picking up 5 gallon buckets of oil and putting them into the manlift basket, the worker injured his right shoulder. The worker went to a physician on 10/9/2009 and was given a prescription for physical therapy. The worker had surgery on 12/23/09.

The worker was carrying a bag when he stepped on the fork of a forklift and twisted his right foot (ankle).

A Bridgeburg employee from the Cowansville Shop was removing the parking brake from a CAT 990 with Come-A-Longs. One of the Come-A-Longs and a nylon strap slipped, and the unit pinched the workers forearm. The worker went to a physician the following day and was put on lite duty.

Fall to the walkway or working surface

The man slipped on ice at bottom of fuel truck and bruised his left knee. He was able to perform his normal duties, but when he was check by doctor, he is on restricted duty which did not interfere with normal work.

The man was loading an oxygen bottle on a truck. His right wrist gave way. Went to emergency room and was put on light duty for a strained wrist. If light duty persists too long, he may miss some time as we do not have much light duty oportunities.

The injured man was descending a short set of steps which he slipped on the rain wetted steps and hit his head. There was nothing broken, but he is suffering from amnesia and will be off work until his amnesia clears.

THE MAN WAS HAULING BRUSH IN ROCK TRUCK. A BRANCH CAME THROUGH HIS SIDE WINDOW PUSHING HIM INTO STEERING COLUMN. NO IMMEDIATE ATTENTION REQUIRED AT THAT TIME. NOW IN OCT. SAID THAT IS AGGRAVATED AN OLD CONDITION AND HE REQUIRED SURGERY AND IS OFF WORK FOR RECOVERY.

EMPLOYEE WAS TAKING AN AGLIME SAMPLE FROM A SAMPLE POINT. THE POINT WAS PLUGGED WITH LIME, HE PUSHED A HOSE IN THE POINT TO UNCLOG INSTEAD OF TAPPING THE POINT. THE HOSE CAUGHT HIS FINGER AND PULLED IT IN THE POINT & HE LOST HIS RT. INDEX FINGERNAIL. EE WAS TREATED AT THE E.R., SAMPLING POINT MOVED TO DECREASE RISK.

THE INJURED MAN WAS MOVING A 55 GAL OIL DRUM. PINCHED RIGHT INDEX FINGER BETWEEN DRUMS TAKEN TO EMERGENCY ROOM FOR X-RAY. NOTHING REQUIRED BUT APRESCRIPTION PAIN MEDICATION WAS GIVEN.

THE MAN TWISTED HIS BACK LIFTING A 50# BAG OF ROCK DUST. NO TREATMENT WAS SOUGHT UNTIL 6-10-02. HE WAS PUT ON RESTRICTED DUTY WHICH DOES NOT INTERFERE WITH HIS REGULAR WORK.

THE INJURED MAN WAS HIT ON RIGHT LEG BY A FALLING ROCK. HE WAS CHECKED AT ER AND FOUND ONLY TO HAVE SUSTAINED A BRUISE. HE WAS GIVEN PRESCRIPTION FOR PAIN AND RELEASED BACK TO WORK.

THE MAN WAS REMOVING THE ACCUMULATOR BOTTLE FROM A CRUSHER WHEN THE BOTTLE MOVED UNEXPECTEDLY AND PINCHED THE LITTLE FINGER ON HIS LEFT HAND. HE WAS TREATED AT THE ER THAT DAY, MISSED WORK THE FOLLOWING DAY AND RETURNED TO REGULAR DUTY THE NEXT DAY.

THE MAN'S HAND SLIPPED ON A FUEL PUMP CAUSING A LACERATION TO THE RING FINGER OF THE RIGHT HAND.FOUR STITCHES WERE REQUIRED.
