Employee was clearing out a blockage on the fines chute underneath the jaw of the primary plant. The employee was using a metal bar to loosen the stuck material. When the last rock came free instead of falling down the chute it came out of the access hatch hitting the miner in the face and hand.

Employee was performing maintenance on a screen deck. Upon completion of work the employee descended down the screen deck ladder. While descending the ladder the employee slipped on a rung and lost footing. To catch themselves from falling the employee caught themselves with left arm subsequently straining the arm and shoulder.

EE was attempting to remove a bearing on screen deck 4 of the rock plant using a Port-a-Power and H-Puller. When pressure was applied the threads were stripped and the H-Brace struck EE in EE's left side rib cage.

Employee was splicing conveyor belt with three other ees at Bellino Tunnel Belt. While positioning belt, ee was standing, slightly bent at waist pulling slack from belt and felt sharp pain in lower back, diagnosed as a strain.

Injury first reported to Foreman on 3/29/17. Date of incident and time are approximate. Employee has burning sensation in left shoulder joint when pulling fuel hose from reel on lube truck to fuel 385 excavator. Employee is still experiencing pain. Possible overuse injury from pulling hose reels many times per day.

During shift miner unknowingly sustained a minor superficial abrasion on EE's right knee. Two hours after shift, EE noticed inflammation, and rapid swelling. At ER EE was diagnosed with prepatellar bursitis, given antibiotics, and released. After 48 hours swelling continued. EE was admitted to hospital for drainage of knee, IV antibiotics, and observation.

Employee was removing pin from side plate on loader bucket. While driving the pin out with a hammer, the hammer glanced the pin and struck a hard-face dot causing the dot to fragment. A metal piece of the dot penetrated the employees leather glove and into his right thumb. Employee was sent to medical office to remove the foreign object.

Employee pinched fingers while pulling a roller into a hose reel on his service truck.

Employee was constructing new conveyor, frame section had no belt, unpowered and no stored energy. While moving frame into place employees hand passed through area between diagonal and horizontal troughing rollers. Space was large enough for employees right hand pinky finger to pass through, but small enough to cause laceration and damage to finger.

Employee was replacing a belt on the Power Screen and bumped his knee, causing a contusion that subsequently became infected. Employee declined medical treatment until 12/13/11. Claim was accepted on 12/13/11 as reportable.

Employee was cutting a belt with a razor when the knife slipped and cut his right lower leg.

Employee suffered Tenosynovitis to the left wrist as a result of striking his wrist against non-sharp angle iron handrail, while walking down stairs at the secondary plant. Employee choose not to seek medical treatment until 2/3/11.

Employee was loosening a bolt off a loader when the wrench broke, came back and hit him in the right eye.

Employee was hammering a wedge into a pin to close the door on the jaw crusher.

Changing plate on jaw. Chain released that was holding plate; plate fell on employee - approx. weight 900 lbs - causing neck, wrist, shoulder contusions.

Employee was replacing a 15 HP electric motor on the crusher feeder. As he tried to position the motor onto the mount, the motor shifted and caught his finger against the frame of the crusher feeder. Employee experienced a laceration on the small finger of his right hand.

DESCENDING FROM A STEP STOOL. EE MISSED THE SECOND STEP, FELL BACKWARD, AND HIT HER HEAD ON THE EDGE OF THE DESK. EE'S IMPACT ON THE EDGE OF THE DESK CAUSED A 3/4" LACERATION TO THE BACK OF THE HEAD.

EMPLOYEE GRASPED A HANDRAIL USED FOR ACCESS TO A PLATFORM UTILIZED TO CHANGE THE BLOW BARS ON A RECYCLE PLANT. EMPLOYEE FELT A SHARP PAIN IN HIS RIGHT SHOULDER. THE PAIN DID NOT SUBSIDE, SO TH E EMPLOYEE WAS DRIVEN TO CALIFORNIA OCCUPATION PHYSICIANS IN MODESTO, CA. FURTHER EVALUATION REVEALED AN ACUTE SPRAIN ON HIS RIGHT SHOULDER.

EMPLOYEE WAS REMOVING ANVILS FROM A CRUSHER UNIT. A PULLING TOOL COULD NOT BE FOUND, SO EMPLOYEE WAS USING A CHIPPING HAMMER TO EXTRACT THE ANVILS. THE HAMMER SLIPPED, PIERCING HIS LIP AND CHIPPING A TOOTH.

EE INFORMED HIS FOREMAN THAT HE DIDN'T FEEL WELL & HAD FELT A PAIN IN HIS RIGHT ARM. FOREMAN TOLD EE HE WAS GOING TO DRIVE HIM TO CALIF. OCCUPATIONAL SERVICES IN MODESTO TO HAVE THE SITUATION CHECKED OUT. EE SAID HE PREFERRED TO GO HOME & CONSULT HIS PERSONAL DR. EE'S DR MET HIM @ SONORA HOSPITAL, RAN SOME TESTS WHICH INDICATED EE HAD MAJOR HEART PROBLEM & HAD EXPERIENCED MILD HEAR

HE STRAINED HIS KNEE WALKING ON UNEVEN GROUND.

HE SLIPPED AND FELL WHILE WELDING ON TEXAS FEEDER AND DISLOCATED HIS RIGHT KNEE.

EMPLOYEE WAS PUTTING HIS TOOLS AWAY. A PIPE JACK FELL OVER AND STRUCK HIS KNEE.

WORKER WAS LOOSENING A NUT ON A CONE CRUSHER UNIT. A WRENCH WAS ATTACHED TO THE NUT, AND THE WORKER WAS HITTING THE WRENCH WITH A 4" SHAFT. WORKER MISSED HIS TARGET AND SLAMMED HIS FINGER AGAI NST THE WRENCH.

EE ATTEMPTED TO CLEAR A BRIDGE OF MATERIAL IN THE PRIMARY JAW CRUSHER WHILE RUNNING WITHOUT AUTHORIZATION WITH A PRY BAR. THE BAR GOT CAUGHT BETWEEN A PIECE OF METAL & THE MOVING SIDE OF THE J AW WHICH CAUSED THE PRY BAR TO STRIKE EE

EE WAS CHANGING THE LIMIT SWITCH ON THE OIL WAY HOPPER INJ EE WAS HOOKING UP SOME WIRE THAT OPENED A OIL VALVE & OVER FILLED THE WAY HOPPER CAUSING HOT OIL TO SPILL OUTO F THE HOPPER ON TO INJ EE

OPERATOR WAS MOVING A HYDRAULIC JACK AFTER ADJUSTING A CONE CRUSHER. HE SLIPPED ON AN OILY SURFACE ON THE CRUSHER PLATFORM, LANDING ON HIS BOTTOM LEFT SIDE.
