Employee was performing maintenance on a screen deck. Upon completion of work the employee descended down the screen deck ladder. While descending the ladder the employee slipped on a rung and lost footing. To catch themselves from falling the employee caught themselves with left arm subsequently straining the arm and shoulder.

EE was attempting to remove a bearing on screen deck 4 of the rock plant using a Port-a-Power and H-Puller. When pressure was applied the threads were stripped and the H-Brace struck EE in EE's left side rib cage.

Injury first reported to Foreman on 3/29/17. Date of incident and time are approximate. Employee has burning sensation in left shoulder joint when pulling fuel hose from reel on lube truck to fuel 385 excavator. Employee is still experiencing pain. Possible overuse injury from pulling hose reels many times per day.

Employee was removing pin from side plate on loader bucket. While driving the pin out with a hammer, the hammer glanced the pin and struck a hard-face dot causing the dot to fragment. A metal piece of the dot penetrated the employees leather glove and into his right thumb. Employee was sent to medical office to remove the foreign object.

EMPLOYEE WAS REMOVING ANVILS FROM A CRUSHER UNIT. A PULLING TOOL COULD NOT BE FOUND, SO EMPLOYEE WAS USING A CHIPPING HAMMER TO EXTRACT THE ANVILS. THE HAMMER SLIPPED, PIERCING HIS LIP AND CHIPPING A TOOTH.
