Front End Loader operator was approaching a pile of sand to fill bucket and place on the screen. The operator said he hit the pile of sand to fill the bucket and he felt pain in his elbow. He kept working and said that his elbow would hurt when moving it. He then reported the incidence to his supervisor. There were no witnesses to the incidence.

Showing all 2Front End Loader operator was descending ladder to take a break when he rolled left ankle as he stepped onto bottom rung. Ladder and operator's boots were both clean and free of debris. Operator stated he maintained three points of contact as he descended ladder. Operator could not recall looking at rungs as he descended. There were no witnesses to accident.
