The Feeder at the primary plant was jammed. When looking for the cause of the jam, the primary plant operator fell off of the step ladder EE was on. EE's hard hat fell off and EE's head hit the concrete. EE lost consciousness for roughly 90 seconds. Ambulance was called. EE's injuries were a subdural hematoma and an open fracture of right side of occipital bone.

Showing all 6He was cutting with a torch and a spark went into his left ear. He went to the dr and returned to work in the same day. nothing was done from then until 11-29-05 when a 15 minutes operation was performed to correct his burn on the ear drum.
WAS WORKING ON DRY SCREEN. WAS HITTING A PUNCH TO LOOSEN A SIDE NAIL USING A 3 LB HAMMER. MISSED THE PUNCH & HIT SMALL FINGER ON LEFT HAND.
WINDOW CAME UNLATCHED WHILE EXITING MACHINE. FINGER CAUGHT BETWEEN WINDOW AND LEDGE, CUTTING AND BREAKING RING FINGER ON RIGHT HAND.
STEPPED OFF OF LOADER AND TWISTED LEFT KNEE. EE ORIGINALLY DID NOT FEEL THE NEED TO GOT O A DR. DI HOWEVER GO TO THE DR. ON 9-12.
WAS GETTING OFF A STEP LADDER AND MISSED A STEP.LANDED FLAT ON HIS LEFT HEAL, DRIVING HIS HEAL INTO THE FLOOR.
