I've been gone since Feb returning on April 5th so this didn't get reported. EE was helping to align a shim under a pillow block bearing when the pry bar slipped while tightening the belts causing the pillow block bearing to drop on EE's left thumb. EE has a non displaced fracture of distal phalanx and laceration without foreign body damage to nail.

EE says EE was doing clean up work with a shovel in the tunnel and pulled a muscle around EE's Rt rib/chest area, but then says it could have happened when EE was mowing the lawn?
EE states was cleaning a conveyor table with a T-rake, the T-rake got into the roller pulling EE's hand into the roller. EE went to the ER, X-rays were taken and all was negative and no restrictions. EE went back to our work comp clinic on May 17th and was given restrictions to also start physical therapy on May 22nd.
EE worked on Saturday in the tunnel welding and moving metal pieces and felt something in right eye but didn't want to go to our work comp Doctor, EE went on own to EE's own Dr and didn't turn it in until EE received the bill. EE had a metallic foreign body in the right eye.
EE was moving steel plates to center grips for the forks, the plate slipped off falling onto right foot. X-ray showed a small fracture.
EE was hammering off a bearing when a piece broke off and flew up into his face cutting his lip. He received four stitches and a chipped tooth.
EE had removed a cover to clean out a hopper, when extending the shovel into the hopper and removing material he was cut by a sharp piece of metal cutting through his gloves.
Working on crusher and fell off backwards. Continued to work that day, but the plant was shut down and employees were laid off including injured employee. On July 31 he came to plant and told me about his accident on July 28 and that he needed to go to the ER. The hospital said that he has a sprain and to keep ice on it.

EE WAS STEPPING OUT OF A DUMP TRUCK, HE STEPPED OUT ONTO SOME LOOSE MATERIAL TWISTING HIS RT ANKLE AND HEARD IT POP. THE CLINIC DID AN XRAY, IT DIDN'T SHOW A FRACTURE. THE DR STATED IT WAS A SPRAIN.
EE was prying on a crusher housing to set it back in place. He was standing on a wooded board covering a work hole used for maintenance on the crusher. The board broke and he fell through the hole catching himself with his arms chest high with his left leg/foot landing on an I-beam. The employee sustained a sprained left shoulder, leg/knee and ankle.
A CONTRACTOR WAS INSTALLING KEY BRICK IN A ROTARY KILN. A KEY BRICK SPRUNG OUT HITTING HIS LEFT INDEX FINGER. THE INJURY CONSISTED OF STITCHES AND FRACTURED THE TIP OF HIS LEFT INDEX FINGER.
EE was getting ready to install a bearing. He squatted down to pick it up and his right knee popped. He notified a supervisor the same day, but didn't want treatment at that time. On March 31st his knee was no better. He then was sent to the clinic. Diagnosis is a Knee Impingement with restrictions.
EE states accident was reported to management on 3/18/14. EE and two others were lifting a 1x6x20' metal bar when he felt a sharp pain in his abdomen. He went to OP Regional ER on 3/17/14 and left with restrictions, but I was not notified.
For about three days the EE had been working in a confined area on his knees bent over at times using a Mag Drill to place steel plates to the wall and to secure the adjusters in the holes.
EE WAS INSTALLING AN EXHAUST ON TOP OF A ROAD GRADER, HE SLIPPED AND FELL BACKWARDS ON THE TIRE. THE DOCTORS FINDINGS IS A BACK CONTUSION AND SPASM OF MUSCLE.
EE was doing his cleanup work, shoveling material and picking up and stacking brick, while doing so he felt a sharp pain in his lower abdomen. The Doctor says he has a Hernia NOS. He is to see a General Surgeon.
EE was getting ready to go down stairs, he tripped on the top step falling down approximately 7 steps to the landing. He fell with his right leg bending backwards underneath him. The Dr put him on crutches and sit down work only until he has an MRI on Nov 14th.
Cutting on conveyor belt and knife slipped and cut index finger on left hand. 5 stitches.
EE was laying underneath a dump truck installing a starter. A piece of metal fell into his left eye. A foreign body was removed leaving a residual rust ring which has to be cleaned out by an Ophthalmologist MD. A Tetanus shot was also given.
EE was getting ready to replace a fan belt. He didn't close the fan louver's and put a board in the fan to keep it from turning as a safety, the fan started turning backwards he tried to stop it with his right foot. The belt pulled his foot between the belt and sheave. It broke both bones in his ankle.
Employee was walking towards the shop door and instead of turning the door knob to open it he hit the door with fist to push open and it broke his hand. Should have used common sense.
Employee was stepping down the ladder of a Highloader , his Rt foot got caught on the last step causing him to step down on his LT foot sideways. He sprain his Lt Ankle. They put him in a boot when he is to be active. He was told to take over the counter meds for pain.
Un-plugging the feed gate when arm slipped and got cut.
Employee Arc welded from 10am - 5:30pm.
Employee reported he fell 3/4/2011. At that point he was okay but sore. Monday A.M. his Rt shoulder was hurting and wanted to be seen. He stated that he was carrying a oxygen bottle when he slipped and fell in the mud falling forward and catching himself with both arms, mostly with the right arm, but also stated his left shoulder was sore also.
EE was jackhammering kiln brick and concrete out of a kiln. He stumbled over the pieces laying on the ground. He sprained his Lt ankle.
Employee states he was moving lifters at the Kiln while doing maintenance work and twisted his back. On Tuesday, November 10th he said his back wasn't any better, he wanted to go get checked out. The Doctor put him on modified duty with therapy.
Employee states he was pulling down on a ratchet with a cheater bar, as he was pulling down on the ratchet it slipped off the nut and it came back on his right finger tip. He sustained a Distal Phalanx (fractured finger tip)
Employee reported to Plant Manager that he kinked his back again by moving a crusher roll. At this time I have not spoken with employee or received a work status report. Employee stated he did not use the electric hoist that is provided.
Walking up stairs and lost footing. Hit forehead on metal piece. Went to ER. Doctor cleaned face and put glue on cut to stop bleeding.
Employee was taking a chunk of shale out of bin when left hand pinky got caught in gate. 1/2" of top part of pinky finger came off.
Employee states he was shoveling when his back started hurting. The Doctor is sending him to therapy for one week, then will re-evaluate.
Employee states he was laying on a metal rail replacing rail rubber. He felt a pain in his right hip area.
# 1 feed chute to kiln was plugged with material. Took bar and tried to get material to go through and hit left hand on metal box around top of #1 feed chute cutting index finger. Went to ER got 3 stitches on index finger.
Employee states he was on a forklift moving a couple of crusher rolls. One of them looked like it was going to roll off, so he got off to put a piece of wood under the roll. As he pushed against the roll he felt a pop and a tear feeling in his lower back.
Employee states he was putting bolts in a lifter, he turned, tripped and fell on his shoulder. The Doctor states a strain of his right shoulder.
Employee said he picked up a piece of shaft and felt a pull in his left groin area.
Employee was working on reducer. Trying to take bolt off. Another employee was using crow bar to hold lid open while injured tried to take bolt off when crow bar slipped and lid closed on his finger. Scratched and bruised right index finger.
Employee states he was changing a grinder wheel on a hand grinder, when he flipped the switch it slipped cutting through his glove and cutting the fourth knuckle of his left hand. He received about eight stitches.
Employee states he was helping to replace a belt on the scalper. When they were replacing the guard he got his Rt. hand caught between the motor and guard. He had soreness to his hand. The Doctor put him in a splint and was given Prednisone.
Employee was going up the ladder to get into a L150 High Loader when he felt his thumb pop. On Monday (11/17/08) he spoke with his Supervisor who then sent him to the Clinic. He was diagnosed having a "Mallet Thumb", he will have a pin put in on 11/19/08 for 8 weeks. The nurse said he will have no restrictions from them, only what he feels he can and can't do.
Employee was taking 970 into pit to empty out material when steam went into window of cab and burned his upper torso.
Employee said he was shoveling with his left hand. He felt a pop in his left shoulder and back, causing a left cervical strain and left shoulder strain. This was not reported to Management until 6/30/2008.
He was trying to jerk open the door (instead of using the pry-bar provided) to change from crushing to scalping.
Employee was using a ratchet pulling down to tighten a bolt. He was using his right leg as leverage to balance himself, but his leg slipped injuring his lower left back.

Employee was rodding material above a head pulley. The head pulley caught the rod, twisting & bending his hand.
