Employee was welding in the quarry an received radiant burn to his upper arms. Employee reported the incident on the morning of the 31st an was seen by local medical clinic. Employee was treated and released to full duty but was given a prescription.
Employee was walking while working at the quarry and slipped on level surface and fell to the ground. The employee was taken to the hospital for evaluation and was found to have a left knee strain. The employee was released from care with modified duty and a prescription.
The EE was drilling holes to place anchors in concrete. While drilling the hole, the drill bit struck rebar which caused drill housing to rotate, striking the EE in the mouth.
